DDS is seeking a skilled and experienced Registered Nurse V (Healthcare Coordinator) to work in the Southeast Region's state operated community-based system. The primary responsibility is to coordinate the healthcare delivery system across the Agency. This role will require effective supervision of a team of Registered Nurses as well as identifying areas of need and implementing corrections through training, meetings, and other avenues. Confers with the individual, family, support staff, nursing staff, administrators, PCP's and other clinicians on a regular and on-going basis. Job Responsibilities:(The duties and responsibilities stated are general summary and not all inclusive.) The RN V (Healthcare Coordinator) will determine individuals' health care needs and assist in the development of supports to meet those needs. This position will also coordinate health care services and access to medical care as well as evaluate the effectiveness of health care services. Additionally, the RN V participates in resolving problems/ barriers to accessing health care services in the community. Maintains and coordinates necessary components of healthcare to be in compliance with licensing and certification standards. Provides consultation and Medication Administration Program (MAP) oversight to staff within the MAP program. Identifies the need for and provides health education and training as needed for direct support staff and others, including, clinicians and contracted service providers. Functions as a resource person to all staff within the Agency as well as individuals and their families regarding any healthcare issues. Additionally, the RN V is a resource to all staff with regards to Agency policies and procedures, MAP, implementation of the professional oversight model, DDS Life Sustaining Policy, and nursing practices as they relate to MAP. Participates as a member of the Standard Operating Procedure committee, Risk Management, Healthcare Committee, and the Training Committee. Provides consultation to the management team at the Respite home which supports individuals across the state. PREFERRED QUALIFICATIONS: Experience with Microsoft Office Suite, specifically, Excel/Word Excellent organizational skills Excellent communication skills Please Note: The position is primarily located in Middleboro, MA. This position is required to have On-Call responsibilities which include one full week on-call/3-week rotation. * This position requires presence in residential programs that operate 24-hours/7 days a week. The Department of Developmental Services requires employees in this position to be available when circumstances dictate the need. This position is designated as an "EMERGENCY" position in the Department of Developmental Services Emergency Plan. This means that whenever the Governor declares that the Commonwealth is in a state of emergency, you will be required to report to work. *This position requires schedule flexibility and travel throughout the Southeast Region. SCHEDULE INFORMATION: Full-time – 40 hour weekly position First Shift -8:30am – 5pm Monday through Friday. Will be scheduled to work one evening per week, 11:30AM – 8PM. SPECIAL REQUIREMENTS FOR ALL LEVELS IN SERIES: Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing . Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination.
