JOB REQUIREMENTS: Care Coordination Assistant - Behavioral Health - 34083 DESCRIPTION/RESPONSIBILITIES: Work Schedule : 100% FTE, days. We are seeking a Care Coordination Assistant to: * Partner with members of the care team to coordinate patient care, navigate patients through the health care system and provide support for patients receiving specialized behavioral health care. * Provide emotional support and education to patients and families within their specialty care discipline. * Collaborate with patients and their families to ensure the patient is appropriately scheduled, and facilitate access to clinical, social, and community resources.
