OVERVIEW OF PRIMARY RESPONSIBILITES :

Partners in Recovery Project (PRP) is an Intensive Apartment Treatment service that provides housing services to a maximum of 18 individuals age 18 or over with a diagnoses of serious mental illness. PRP practitioners provide the individual served with supports in the domain areas of their choosing.

PRIMARY FUNCTIONS :

  • Perform assessments

  • Develop service plans

  • Provide restorative services and crisis intervention

  • Be responsible for timely documentation in accordance with NYS OMH and Medicaid regulations

  • Carry a caseload of 3 to 5 individuals and be on-call to respond 24/7 to these individuals as needed.

    ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT :

    The PRP Practitioner will demonstrate clinical therapeutic skills and the ability to utilize them in nontraditional ways. They will understand existing service systems within the community and will be skilled in identifying appropriate resources. In this position, an individual will be responsible for the development of a partnership with individuals in the program, while working with the broader service team to develop natural supports.

    QUALIFICATIONS AND ATTRIBUTES :

    The ideal candidate will be enthusiastic, engaging, be able to multi-task and have excellent organizational, communication, and computer skills.

    EDUCATION AND EXPERIENCE :

    Minimum BA degree with life Recovery Experience Preferred and at least 2 years’ experience working in behavioral healthcare

    These physical demands are representative of the physical requirements necessary for an employee to perform the job's essential functions successfully.   • Must be capable to sit or stand in front of a computer for long-periods of time

  • Able to work in open space floor plan

  • Must be capable to move throughout work day (and follow individuals throughout the community)

  • Work alongside co-workers within 3 feet

  • Must be able to move in tight spaces

  • Occasional lifting of > _25+pounds

    PERCENT OF TRAVEL: 50%
