PharmacistJob Description

Provides pharmaceutical services to long-term care and institutionalized patients in a manner that maximizes quality and patient safety. Participates in and oversees the dispensing of medication in accordance with federal and state regulations.

Responsibilities

  • Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Make recommendations for therapy changes to the physician and facility staff as appropriate.

  • Maintain patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).

  • Ensure that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with appropriate members of the healthcare team.

  • Collaborate with the healthcare team to provide clinical and technical support as needed.

  • Document all communication with physicians, nurses, other members of the healthcare team, and patients/residents and their family members in the medical record.

  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.

  • Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and company policy.

  • Inform the Pharmacy Manager of medication-related errors.

  • Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

  • Perform other tasks as assigned.

Essential Skills

  • Licensed Pharmacist in Wisconsin

  • Experience in long-term care (LTC) pharmacy preferred

  • PharmD degree

Additional Skills & Qualifications

  • Strong knowledge of federal and state regulations related to pharmacy

  • Excellent communication and documentation skills

  • Ability to collaborate effectively with healthcare team members

  • Attention to detail and ability to manage multiple tasks

Work Environment

The role is based in a long-term care pharmacy. 2 fulltime permanent openings! One position would be 9:00 AM to 5:30 PM, and another position requires working from 1:00 PM to 9:30 PM. Both positions will require working some Saturdays. The work environment is collaborative, and you will interact with various healthcare professionals to ensure patient safety and care.

Pay and Benefits

The pay range for this position is $65.00 - $65.00
