FTE: 0.001000 Shift: 1 Schedule: optional part time position picking up hours Monday- Friday 0800-1630 Job Summary: Responsible for facilitating the care of designated patients and their families during a patient\'s hospital admission or outpatient visit. Coordinates discharge plan from pre-admission through discharge follow up. Provides psychosocial assessment and updates that address the patient\'s evolving needs. Establishes a plan of care that provides maximum benefit to patient and their families. Other duties as assigned. EXPERIENCE DESCRIPTION: Minimum of two years experience in an acute care setting preferred.EDUCATION DESCRIPTION: Masters Degree in Social Work with casework in medical social work.SPECIAL SKILLS DESCRIPTION: Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients, families and hospital personnel.
