This bonus will be paid in 3 separate checks in the following manner. \$1,500 of the sign-on bonus after 30 days of employment. \$1,500 of the sign-on bonus after 90 days of employment. \$4,500 of the sign-on bonus after 6 months of employment. Position Summary: This position is responsible for clinical oversight of a team of CCS Care Coordinators and Ancillary Providers. Working under the Wraparound and Youth CCS systems of care, which includes a unique set of values that emphasizes individualized, strength-based, trauma-informed, family-driven, community-based, and culturally sensitive care. Essential Functions: Provide clinical supervision and management to CCS staff. This includes individual, group and side by side supervision in accordance with DHS 36. Assist staff in identifying services from the CCS service array appropriate to address barriers to recovery. Monitor and approve staff documentation to ensure good clinical practice and maximize billing. This includes but is not limited to Provider Notes, CCS Assessment Summaries, and Plans of Care. Oversee CCS discharges in collaboration with Care Coordinators and Milwaukee County Wraparound Administration via review of Discharge Summaries and ensure that discharges are completed according to DHS 36. Ensure accountability for timely annual functional screen evaluations which determine ongoing eligibility for Youth CCS enrollees. Assist with completion of annual functional screen evaluations as needed. Attend safety plan meetings, team meetings, and/or other face to face visits for high-risk youth, as requested by Care Coordinators, to provide clinical assessment and support. Keep Wraparound Milwaukee/Youth CCS Administrators informed of emergency or critical incidents in collaboration with Program Manager and Assistant Program Director. Attend all required trainings and in-services as requested by Milwaukee County and Wisconsin Community Services. Assist and support CCS staff during crisis situations/provides coverage during times of staff shortage.
