HealthPartners Case Manager, Employee Health Services in Minneapolis, Minnesota

HealthPartners is hiring a Employee Health Services Case Manager. This position is responsible for assessing needs, coordinating services, providing support and guidance to all HealthPartners colleagues across the Organization in addressing colleague health and safety needs. This includes proactive Employee Health Services programs support, incidental issue intake and management to resolution by way of various routes such as phone intake and outbound calls, voicemail, email, online chat, etc. This role will balance various stakeholder needs to each unique situation (e.g., colleague, colleague's leader, organization, OSHA, MDH, Joint Commission etc.). A successful individual in this role will take ownership to ensure colleagues have resolution by providing support and guidance and connect colleagues with appropriate resources. ACCOUNTABILITIES: Responsible for conducting colleague exposure investigations Evaluates colleagues' risk of exposure Provides nursing expertise for worker's compensation program Assesses colleague's return to work status Coordinates with applicable stakeholders Provides nursing expertise for ergonomic program Assesses medical questionnaires submitted as part of the request for an ergonomic evaluation Provides nursing expertise for the respiratory protection program Assesses medical questionnaires submitted by colleagues for fit testing Provides nursing expertise for medical surveillance programs Assesses medical questionnaires provided by colleagues enrolled in the program Provides nursing expertise for the colleague vaccine program Educates colleagues regarding vaccine safety Provides nursing expertise for colleague leaves of absence Evaluates ADAAA medical questionnaires and reasonable accommodation requests Evaluates FMLA medical certifications Provides nursing expertise in new hire screenings Evaluates colleagues' lab results Colleague focus Views and cares for those we serve as the center of all we do. Makes interactions that colleagues have with Employee Health as simple as possible for them. Is a confident guide to support colleagues in situations that are complex and stressful. Identifies interventions and resources to assist colleagues in reaching employee health related goals. Identifies patterns, episodes, and gaps that are predictive of future needs and services. Protects the sensitive nature and exposure to colleague's health records and balance with regulatory and organizational requirements and expectations around sharing with diligent confidentiality. Integration Integrates clinical and psychosocial information for colleague interactions to develop action-oriented and time-specific planning and implementation of appropriate interventions. Facilitates integration of care by encouragement of effective communications between colleagues, leaders, providers, and other relevant parties. Works with colleague, leaders and other departments to ensure compliance with organizational and outside agency requirements and regulations. Communication Effectively communicates with colleagues, leaders, and other team members, adapting communication to audience needs. Maintains current and accurate documentation Maintains confidentiality of information in accordance with department and organizational policies. Team approach Works collaboratively to ensure those we serve are cared for in a timely, compassionate, effective manner. Technology Maintains knowledge of and effectively uses Information Technology applications and systems. Identifies deficits in technological literacy and seeks appropriate training under guidance of supervisor. Other Duties Improving quality is everyone's responsibility. Identify improvement opportunities and use quality improvement methodologies to make improvements and contribute and assist others in doing the same. Performs other duties as assigned. Is flexible with schedule, as this role may include evening, weekend, holiday, and on call coverage to meet the needs of those we se ve. REQUIRED QUALIFICATIONS: Current Registered Nurse in the State of Minnesota Current Registered Nurse in Wisconsin within 2 months of hire BSN or three years RN experience Demonstrated effective, independent nursing judgment and skills. Demonstrated critical thinking and problem-solving skills. Demonstrated skill and experience in effectively collaborating with care team members, using a high level of expertise in written, oral and interpersonal communication. Demonstrated working knowledge of quality improvement. Demonstrated flexibility, organization, and appropriate decision-making under challenging situations. Demonstrated approaching work by putting those we serve first. Robust computer systems skills, including knowledge of Microsoft suite, and eagerness to learn new systems, including formal and self-directed learning. PREFERRED QUALIFICATIONS: Previous experience as a case manager or similar role in Employee Health, Occupational Medicine, or related area. Master's degree in related field. Certified Occupational Health Nurse (COHN). DECISION-MAKING: Makes independent decisions within the scope of this position's accountabilities and determines the need for and the timing of consultation with leadership and/or Medical Director. Uses professional clinical judgment, organizational knowledge, industry knowledge, and critical thinking skills in determining appropriate plans for colleagues, consulting with leadership and/or Medical Director, when indicated. Makes recommendations to leadership regarding policy development needs and/or changes.
