Actalent MTM Pharmacist - Fully Remote in Minneapolis, Minnesota

Job Title: Pharmacist

Job Description

As a Pharmacist, you will perform comprehensive medication therapy reviews with eligible members, identify, prevent, and resolve medication-related problems, and document the member encounter in the MTM platform. You will conduct encounters with members to improve outcomes related to medication adherence, gaps in therapy, and other measures. You will also contribute to projects supporting business needs and collaborate with team members in MTM/CCM support roles.

Responsibilities

  • Perform comprehensive medication therapy reviews with eligible members.

  • Identify, prevent, and resolve medication-related problems to maximize benefits and minimize costs.

  • Document member encounters in the MTM platform.

  • Complete outreach to other healthcare professionals as necessary.

  • Conduct encounters with members to improve outcomes related to medication adherence, gaps in therapy, and other measures.

  • Contribute to projects as needed to support business needs.

  • Collaborate with team members in MTM/CCM support roles by providing assistance and guidance as needed.

  • Perform other duties as assigned.

Essential Skills

  • Minimum of 1 year as an MTM Pharmacist.

  • Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree.

  • Current pharmacist license in good standing with the State Board of Pharmacy.

  • 1+ years of MTM-specific experience (ideally with a PBM or insurance provider)

Work Environment

This is a remote position with hours of operation from 8:00 AM to 5:30 PM EST. Training will also be from 8:00 AM to 5:30 PM EST. Candidates can be located in any state except California, Hawaii, or Virginia.

Pay and Benefits

The pay range for this position is $55.00 - $55.00
