CUHCC is building a care model that seeks to improve care at four different points: before the visit, during the visit, after the visit and between visits. The Referral & Orders Coordinator will be responsible for providing care after the medical or psychiatry appointment has been completed. The initial focus will be to help prioritize and serve those who are at high risk and who need immediate follow up upon completion of the visit to ensure that they have a path to the next steps in their care. This position will work independently and alongside the providers, interpreters, nurses and other members of the patient's care team to achieve care goals. The Referral Coordinator will also follow all UMN and CUHCC policies, procedures, protocols, and manuals as it applies to the duties and assures compliance and other regulatory requirements. This is a 20 hour/week, 4 hours per day position on-site at CUHCC. Hours for this position are Monday - Friday ranging from 7:30am - 5:30pm. Pay for this position starts at $22.36 per hour depending on experience. Essential Functions Referral Coordination: (95%) Ensure all external & internal referrals and orders such as Durable Medical Equipment (DMEs), Prior Authorizations, Health Home Care re-certifications, are complete and properly routed to the appropriate provider. Assist in scheduling follow up appointments and/or referral appointments to other facilities per provider request. Assist patients with maps, facility location and direct dial numbers. Coordinate with colleagues/Health Information Management to organize and ensure that all required records, imaging, and/or procedural results are available to the external provider before patient's appointment. Assist the patient and/or the care team to ensure patients receive assistance for medications, find transportation, or connect to community resources (food shelves, groups, shelters), as needed. Assist providers in completing patient forms i.e. Medical Opinion forms, Supplemental Security Income (SSI) Disability forms, etc. when appropriate. Assist patient in returning to the patient service representatives to apply for public programs, if needed. Work collaboratively with staff in other patient care areas including Health Information Management, Compliance, Registration, etc. Attend all staff meetings. Participate in quality improvements, as assigned. Other duties as assigned. (5%) CUHCC reserves the right to change or add duties to this position consistent with the job classification. Qualifications Required Qualifications Graduate of an approved Medical Secretarial program; or one year of medical clerical experience which must include keyboarding experience and experience interacting with people (i.e., receptionist, switchboard, etc.) 3 years of customer service experience. Experience working with electronic medical records and other software such as Microsoft Office Suite or Google Suite. Preferred Qualifications Fluency in one of CUHCC's predominant patient languages (Spanish, Somali, Vietnamese, Lao, Hmong, Pashto, Dari). Knowledge and interest in serving multi-culturally diverse patients with mental, medical, dental, sexual and chemical health needs. Experience working in a community clinic. A negative TB test and Hepatitis B Vaccination are required for this position. About the Department CUHCC serves nearly 11,000 patients a year through over 55,000 visits annually.
