On Call Clinicians RN Case Manager in Minneapolis, Minnesota

On Call Clinicians is in search of an experienced RN Case Manager for homecare to work out of an Edina office and see patients in the cities of: Woodbury, St. Paul, and Maplewood. Accountable To: Home Care Clinical Manager Primary Objective: Provide nursing assessment, skilled nursing care, and case management to ill, elderly, and disabled individuals in their place of residence. Knowledge and skills required: 1. Knowledge of professional nursing practices and principles 2. Knowledge in developing nursing care plans 3. Knowledge of current methods in pain and symptom control 4. Ability to adapt nursing principles and practices to a variety of home settings 5. Knowledge of current methods in chronic illness, terminal illness and geriatric nursing 6. Skill in written and oral communication 7. Demonstrates excellent interpersonal skills and team working skills 8. Adaptable to a wide variety of changing situations 9. Knowledgeable with use of a mobile device. Can demonstrate keyboarding skills necessary to perform clinical charting within an Electronic Medical Record 10. Ability to prioritize and organize work effectively and efficiently to accomplish workload in the allotted time. 11. Ability to facilitate positive morale with co-workers, clients, families and other professionals 12. High degree of professionalism when working with staff, clients and public 13. Maintain always client confidentiality as per the Health Insurance Portability and Accountability Act (HIPAA). Develop and demonstrate a set standard of personal expectations for excellence in quality and quantity of work. Essential Duties: 1. Attendance for reliability for clients to receive uninterrupted services 2. Provides skilled nursing care under physician orders. 3. Provides nursing assessment, develops and executes plan of care interventions, evaluates response to interventions for each individual patient and updates physician/plan of care as needed to gain positive outcomes. 4. Promotes, maintains, and restores health through teaching, counseling, and use of appropriate preventative, rehabilitative and /or palliative measures, and with ongoing observation and assessment of response. 5. Collaborates with the patient s physician regarding the plan of care and arranges/assures the Face to Face Encounter with PCP is completed as required. 6. Works with billing to manage timely documentation for the purpose of attaining insurance authorization timely. 7. Assures quality of care is maintained and client s needs are appropriately met. 8. Maintains and completes accurate documentation in timely manner. a. Point of care documentation will be completed with the expectation that at least 80% of the documentation is done in the patient s home. b. Admission/Recertification/Resumption of Care/Significant Change in Condition assessment visits should be completed and synced into database within 24 hours. (me) c. Transfer/Discharge visits should be completed and synced in on the same day. d. Subsequent visits should be completed and synced in on the same day 9. Schedules/completes visits during regular business hours and per weekend/holiday rotation schedule. 10. Participates in case conferences and/or IDT meetings 11. Attends staff meetings and educational programs as required. 12. Follows State and Federal Regulations for Home Care and Hospice 13. Follows all safety policies and procedures. 14. Case manages home care needs of 25-30 patients to provide a safe care plan, using all available agency and community resources. 15. Completes 5-6 visits per day, on the average. 16. Delegates personal care or homemaking duties to home health aides and homemakers. Participates in instruction, supervision and evaluation of aides and homemakers 17. Delegates visits to LPN. Participates in hand-off, instruction, supervision, and evaluation of LPN. 18. Assists with Quality Assurance Performance Improvement (QAPI) and Process Improvement Projects (PIP) as requested 19. Other duties as assigned 20. Involves rotating through schedule as Second Tier On-Call Nurse to meet the needs of patients who may need unplanned/unscheduled visits outside of business hours. Reliable Transportation Necessary Work Environment: 1. Requires daily travel by automobile in all weather conditions 2. Occasional exposure to unclean or unsanitary conditions during course of home visits including cats, dogs, smokers. Physical Demands: This position involves sitting, driving, standing, walking, repeated bending, squatting, stooping or similar strenuous duties, hand repetitive movements and viewing a digital monitor. This position also involves lifting to 50 lbs. occasionally. We are looking for a qualified candidate to start immediately!
