STATEMENT OF RESPONSIBILITY The LPN is a licensed professional who renders nursing care to clients/patients; functions with a high level of sophistication in the observation of clients/patients and professional judgment; delegates nursing functions in accordance with the Nurse Practice Act and level of staff capabilities; provides direct and indirect patient care services primarily delivered on a visiting and intermittent basis in the patient's/client's home under a physician's plan of treatment and in accordance with the policies andprocedures of the Agency. REPORTS TO: Agency Director, Nurse Manager QUALIFICATIONS:

1. Licensed as an LPN in the state of Alabama
2. Is a graduate of an accredited school of professional nursing
3. Minimum of two (2) years of clinical experience in public health, hospital or long term care setting
4. Good physical and emotional health.

DUTIES: May include but are not limited to the following:

Assesses the needs of patients and assigns nursing care functions accordingly

Performs nursing functions requiring LPN knowledge and skills

Continually re-evaluates the patient's nursing needs.
