Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will assist in the assessment, planning, implementation, and evaluation of patient care. The LPN will collaborate with the care team, participate in interdisciplinary care, and advocate for patients, ensuring they receive person-centered care and services.

Key Responsibilities:

  • Assist in the assessment, planning, implementation, and evaluation of patient care

  • Collaborate with the healthcare team to provide comprehensive care.

  • Participate in interdisciplinary care meetings and contribute to care planning.

  • Advocate for patients’ needs and preferences.

  • Provide person-centered care and services, ensuring the dignity and respect of each patient.

  • Take patients’ vital signs and report any changes in patients’ or residents’ conditions.

  • Coordinate care with the interdisciplinary team.

  • Document appropriately and thoroughly through the EMR/Point Click Care.

  • Keep nursing personnel and the responsible RN informed regarding changes in the conditions of patients/residents.

  • Monitor patients’ conditions and report any changes to the healthcare team.

  • Educate patients and their families about health conditions and care plans.

  • Maintain accurate and timely documentation of patient care.

  • Participate in unit councils and staff meetings.

Qualifications:

  • Current LPN license in good standing in New York State.

  • Strong communication and interpersonal skills.

  • Ability to work effectively in a team environment.

  • Compassionate and patient-focused approach to care.

  • Experience working as an LPN
