Responsible for performing the functions that enable patients to increase function and mobility

Principal Accountabilities:

1. Assists patient with activities of daily living.

2. Turns and positions patients per policy or as directed and transfers patient to and from bed/chair/stretcher.

3. Assists patients with ambulation utilizing the appropriate assistive device and providing the appropriate amount of assistance.

4. Adheres to hospital policies and procedures of care.

5. Observes and maintains isolation precautions.

6. Remains actively involved with assigned unit and rehab department to reduce hospital acquired infections by observing isolation precautions and practicing hand hygiene.

7. Maintains standard precautions and follows isolation instructions.

8. Assures appropriate PPE supplies are utilized as needed.

9. Records patient contact timely and accurately.

10. Documents ADLs and other pertinent information in a timely manner.

11. Completes verbal and written reports by end of shift.

12. Other duties as assigned

Required:

1. High School Diploma

Preferred:

1. BLS certification preferred

2. Prior acute care experience preferred
