County of Muskegon Comprehensive Jail Assessment Specialist in Muskegon, Michigan

The Comprehensive Jail Assessment Specialist under the direction of a Program Supervisor, serves to facilitate screening and assessment for individuals in custody, support the coordination of care and facilitate any initial workflow process of intake and ensure successful entrance onto a treatment team for consumers per medical necessity. The Comprehensive Jail Assessment Specialist is responsible for collecting clinical information, synthesizing needs and strengths in a concise report format, and facilitating forward movement to an appropriate treatment team. The Comprehensive Jail Assessment Specialist may link consumers to care opportunities available while in custody and to community resources based on short- term need. Assessments occur by scheduled appointment and may be in person or through a telehealth format.
