GENERAL SUMMARY AND PURPOSE:

Reporting to the SAHS Manager, Pharmacy Clinical Services, this position is responsible for assisting in developing, implementing, and maintaining the Antimicrobial Stewardship Program inclusive of both the inpatient and ambulatory areas for the health system. You will have the opportunity to develop programs, consult with a multi-D team, prepare and feature presentations, and educate / mentor residents.

The incumbent will work cooperatively with department, hospital, medical group and medical staff to review, identify, design and implement medication use processes in regard to antibiotic stewardship and meet Joint Commission, federal and state regulatory requirements.

This position will include five working days, M-F. Daytime hours only.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

Doctor of Pharmacy or Bachelor of Science in Pharmacy required. Must be licensed as a Pharmacist with the Idaho and Oregon State Boards of Pharmacy. In depth knowledge of local, state and federal laws and regulations related to pharmacy and medication use processes is required.

PGY1 pharmacy practice residency training (or equivalent experience) required.

PGY2 residency training in Infectious Diseases/Antimicrobial Stewardship preferred. Board Certification in Infectious Diseases Pharmacotherapy (BCIDP) preferred. Experience in implementing inpatient and ambulatory care stewardship programs preferred.

Experience in submitting data to the National Healthcare Safety Network (NHSN) Antimicrobial Use and Resistance (AUR) Module preferred. Knowledge of organizational and Joint Commission standards for pharmacy services and Antibiotic Stewardship. Ability to gather information, interpret reports, define strategic initiatives, and guide pharmacists and providers to accomplish those initiatives.

ESSENTIAL FUNCTIONS:

Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. Establish and expand clinical services in the area of infectious disease/antimicrobial stewardship. Clinical surveillance of anti-infection medication use data.

Project development necessary to support the organization’s antimicrobial stewardship goals and initiatives.

Work with related programs and committees to optimize the use of anti-infectives across the health system. Provides mentorship and is a clinical resource for staff, pharmacy students, pharmacy residents and other healthcare providers in the area of infectious disease/antimicrobial stewardship.

Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, Geriatric as applicable).

Revenue management: ensures the accuracy of documenting services and supplies provided to the patients. Reviews patient history incorporating various pharmacy activities providing customers with adequate education for optimum outcomes.

Prepares pharmaceutical products for patients’ use by safe and cost-effective methods using acceptable professional standards. Comprehends and supports quality assurance and risk management issues. Participates in various departmental duties; material resource management, clerical and financial responsibilities, safety, and other issues. Supervises technicians and pharmacy support staff. Lead and conduct meetings with multi-disciplinary care givers.
