[Core Health Partners (CHP) is seeking full-time and Part-time Physical Therapy Assistants to fulfill the following job duties: 1. Treat patient needs and follow treatment plans of supervising therapist 2. Provide physical/aquatic therapy treatment services 3. Instruct patients with chronic disease conditions, physical disabilities, or injuries with-in the guidance and direction of the supervising therapist. 4. Supervise a team Aides - Student interns as assigned [Patient Population - Work with patients across the lifespan (adult and pediatric), however we are able to customize to preference and skill set. [Work Environment: - Integrated team setting - Clinic-to-community model of therapy. Must be able to adapt, innovate and operate in a non-typical clinical environment. [Professional Relationships - Must be able to develop and maintain a strong relationships with other therapists and the network of referring physicians. [I. Job Responsibilities: - Deliver high-quality patient care services in accordance with professional guidelines and code of ethics - Conduct treatments and follow the personalized plans of the supervising therapist: - Promote movement and function - Reduce pain and discomfort - Prevent disability and disease progression - Collaborate with patients, family members, and healthcare providers to achieve optimal outcomes - Follow the direction of the supervising therapists to deliver comprehensive treatment plans - Provide physical and aquatic therapy treatment services under the direction of the evaluating therapist for various conditions, including: - Musculoskeletal conditions (e.g., back pain, joint injuries) - Developmental delays (e.g., pediatrics, neonatal care) - Chronic diseases (e.g., diabetes, obesity, Parkinson's, arthritis, Alzheimer's, vascular disease) - Provide communication in the form of clean and accurate clinical notes and verbal [Job Description- Core Health Partners [communication to the supervising therapists on the patient treatments to achieve maximum functionali y and independence, empowering them to maintain wellness goals and a clear path to long-term success after discharge. Data Management and Reporting - Collect and report data to support patient care and outcomes - Under the direction of the supervising therapist you will be expected to follow instructions and deliver reports to assist caregivers and community non-profits in continuing patient care beyond medical billing [III.
