Job Summary and Qualifications

To deliver the successful operation of the Clinically Complex High Risk patient. Oversees all processes of Palliative Care and conversion of Hospice patients. Including, but not limited to Long-term symptom management, diagnosis of COPD, Stage IV Cancer, and End of Life patients.

  • Monitors clinically complex and disease specific patients for appropriate plan of care; including discharge plan, palliative care, hospice referrals, readmission avoidance, social work triggers, and appropriateness for next level of care for continuity.

  • Identify and address any obstacles, and barriers to timely and quality patient care.

  • Assist with execution of Palliative Care assessments, consults, and order execution.

  • Physician and staff education related to Hospice and Palliative Care processes

  • Liaison between patient and primary care physician for continuation of care.

  • Assists medical staff to improve DNR on admission documentation, and direct admission to Hospice when appropriate.

  • Educate and ensure that Hospice/Palliative Care triggers are in place and utilized hospital wide.

  • Ensures timely conversion of patients from acute care to Hospice Inpatient (GIP)

  • Execution of critical thinking skills to expedite the progression of patients desired outcomes; developing long and short term plans refer cases as needs are identified for optimal patient outcomes.

  • Communicate with patient/family, multidisciplinary team, and medical staff regarding plan of care, benefits and status.

  • Communicating to physician, discharge planners, when patients have a significant change in status.

  • Works with interdisciplinary team to monitor, implement, and achieve best possible outcomes for the disease specificity. Engages with Nursing/Medical Leadership across the hospital setting to ensure proper care at the bedside. Maintains open communication, especially with Patient Safety Director and CMO, communicating clinical team accomplishments, concerns, difficulties, etc. Attends all necessary meetings and events that may occasionally include community events.

  • Assist in designs clinical guidelines (order sets) for diagnoses/procedures based on standards of care; seeks approval through the appropriate process with Interdisciplinary team approach, Coordinates with Interdisciplinary team to ensure that programmatic goals are met.

  • Meet physician expectations of clinical capabilities related to disease specific area

  • Completes monthly and quarterly data collection, with identification of opportunities for improvement and quality improvement initiatives.

  • Oversees implementation and adherence to established clinical guidelines. Works with the interdisciplinary team to monitor, implement, and achieve best possible outcomes for the disease specificity. Engages with Nursing Leadership across the hospital setting to ensure proper care at the bedside.

  • Customize critical thinking skills to expedite the progression of patients desired outcomes; developing long and short term plans; refer cases to the discharge planner and/or utilization nurse when needs are identified.

  • Adhere to plans, policies, and procedures.

  • Utilizes information to facilitate process improvement to the case management function.

  • Monitor for avoidable days to identify opportunities for throughput; communicating with utilization review and/or discharge planners impending denials or non-certified days.

  • Anticipate and prioritize workload efficiently; displaying independent problem solving skills.

  • Attends Staff Meetings Regularly

  • Graduate of Accredited School of Nursing; CCM preferred. BSN preferred

  • A minimum of two years ICU experience or Palliative / Hospice Care experience.

  • Tennessee RN L icense
