Position Description

The Program Coordinator - Social Services is a position located in the Department of Social Services and is responsible for performing professional duties involved in assisting individuals and customers with overcoming barriers to become self-sufficient and have the best quality of life possible. The incumbent will work with all populations, except for minors, including homeless individuals and families and those at risk of becoming homeless.

Metropolitan Social Services is in the business of helping Nashvillians who are experiencing hardship. Services include solution-oriented counseling, information and referral, case management and other assistance. Typical Duties

  • Assessing eligibility of customers according to established agency policies and procedures.

  • Providing information and referral services to customers and the public in a courteous and timely manner.

  • Providing case management services to all populations served.

  • Making home visits to customers' homes and/or other agencies/locations within the community and/or designated collocated partnership sites.

  • Providing customers with the information that enables them to utilize the programs' services to best address their needs.

  • Multi-tasking and working in multiple case management systems and computer applications.

  • Performing concise, comprehensive social assessments and developing service plans based on the customers' individual needs.

  • Working with the Public Defender's & Conservatorship partnership as it pertains to the case management, implementation and monitoring of the data/outcomes.

  • Working with various established Metro partnerships such as the Office of Conservatorship Management, and the HERO'S program through Metro Nashville Public Schools.

  • Completing a detailed Social comprehensive assessment and all needed documentation and forms completely, accurately and in a timely and ongoing manner.

  • Building working relationships with various Metro Departments and other agencies to ensure the emotional and social well-being of the assigned constituents are being met, assessed and documented on a regular basis.

  • Facilitating program specific groups.

  • Attending meetings and trainings as needed.

  • Placing all data in the case management system accurately and in a timely manner.

  • Participating in MSS sponsored events as well as representing the agency at other community events.

  • Responsible for all weekly, monthly, quarterly and annual reports.

  • Assumes other duties as needed and/or assigned.

Minimum Qualifications

Bachelor's degree from an accredited college or university, and (2) two years of experience in program administration.

LICENSES REQUIRED

Valid Driver License

Note: Per Metro Ordinance No. SO94-1078. All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government.

Preferred Experience, Knowledge, Skills, and Abilities

  • Bachelor's degree in Social Work

  • Two (2) or more years of professional social work or case management experience in the mental health, domestic violence, homelessness and/or substance abuse field

  • Skilled in interviewing, making case assessment, counseling and assisting clients in goal formulation and realizations and case referrals

  • Experience with conservatorship programs and issues

  • Bilingual
