Child & Family Services Health Equity Specialist in New Bedford, Massachusetts

Job description Child & Family Services is looking to hire a full-time, Health Equity Specialist who will be primarily responsible for supporting the development and implementation of the organization?s health care equity strategies and initiatives in accordance with the MassHealth CBHC Quality and Equity Incentive Program (CQEIP). This Health Equity Specialist will support the agency with overall performance improvements in the following domains: demographic and health-related social needs data; equitable quality and access; and capacity and collaboration, ensuring that all health care equity initiatives are inclusive, equitable, and effective. The ideal candidate should have a passion for health care equity, a deep understanding of health disparities, the ability to think strategically, and the ability to build relationships and collaborate with diverse stakeholders within the community. Qualifications: Bachelors? degree in public health, public policy, or a related clinical or administrative area within healthcare or public health required. Masters? degree is preferred. Develop and implement the agency?s health care equity strategic plan and quality improvement initiatives within a value-based program model Collaborate with leadership to promote and integrate health equity across agency policies, programs, protocols, and services Design and implement improvement projects to address health disparities Stay aware of current trends in health care equity and develop innovative solutions to address them Cultivate relationships with diverse stakeholders to ensure successful execution of the organization?s health care equity initiatives Develop and implement effective communication and outreach strategies to ensure stakeholders are informed about the organization?s health care equity initiatives Chair the Health Quality and Equity Committee, including creating agendas for meetings and presenting regular reports to the Community Accountability Board (CAB) Serve as a liaison to the agency?s Community Accountability Board (CAB); recruit and engage our client populations for participation on the board Ensure meaningful access to healthcare services through annual organization self-assessments and action plan development Complete the annual Disability Competence Care Self-Assessment Report (DCCSAR) and develop plans to enhance competency in providing disability-competent care Analyze data from screenings and prepare annual reports on disability accommodation needs Monitor progress towards health equity strategic goals and analyze data to identify areas of improvement Prepare and present written and oral reports containing detailed findings and recommendations Facilitate and/or provide guidance on staff training re: social determinants of health, barriers to care, health equity, and cultural diversity and inclusion to promote health improvement strategies Foster an agency-wide culture of centering policies, procedures and practices that are aligned with principles of health equity Participate in agency CQI chart reviews and other quality improvement initiatives
