Mary Wade is hiring for a Nursing Home Social Worker: Per-Diem/Temporary
Hourly Range $24 to $26: Based on Experience

Qualifications:
Bachelor's degree in Social Work or related field
Minimum BSW, MSW (preferred) or LCSW
1 year of Social Work Experience
Skilled nursing experience preferred
MDS experience preferred
Job Responsibilities:
Work with residents, families and healthcare providers to help residents adjust to the Nursing Home setting in order to receive appropriate services to maximize independence.
Plan, organize and lead individual or group conferences to understand, accept, and follow medical recommendations for residents.
Provide recommendations to help restore resident to optimum well-being within resident’s capacity.
Utilizes community based resources to assist residents to resume life in the community.
Prepare service plan and promotes all rules regarding residents’ rights.
Ability to work a flexible work schedule to accommodate for evening and weekend family appointments.
