Mary Wade is hiring for a Nursing Home Social Worker: Per-Diem/Temporary

Hourly Range $24 to $26: Based on Experience

Our resident population is primarily seniors with varying levels of acuity.

Qualifications:

  • Bachelor's degree in Social Work or related field

  • Minimum BSW, MSW (preferred) or LCSW

  • 1 year of Social Work Experience

  • Skilled nursing experience preferred

  • MDS experience preferred

Job Responsibilities:

  • Work with residents, families and healthcare providers to help residents adjust to the Nursing Home setting in order to receive appropriate services to maximize independence.

  • Plan, organize and lead individual or group conferences to understand, accept, and follow medical recommendations for residents.

  • Provide recommendations to help restore resident to optimum well-being within resident’s capacity.

  • Utilizes community based resources to assist residents to resume life in the community.

  • Prepare service plan and promotes all rules regarding residents’ rights.

  • Ability to work a flexible work schedule to accommodate for evening and weekend family appointments.
