Cornell Scott Hill Health Corporation SBHC Coordinator in New Haven, Connecticut
The Site Coordinator is responsible for coordinating and managing the daily operations of the School Based Health Center (SBHC). This role ensures the smooth functioning of the center, supports staff, and provides excellent service to students and families.
Responsibilities include but are not limited to:
Coordinate daily operations of the SBHC.
Assist in scheduling and maintaining clinical schedules.
Locate and escort clients throughout the school buildings.
Verifies insurance coverage as indicated by CS-HHC billing procedures.
Receives telephone calls from patients and correctly and accurately records and transmits messages for Providers. Initiates outbound calls as needed.
Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary.
Prepares patients and rooms for exams according to specific procedures.
Accurately and legibly documents vital signs, including blood pressure, pulse, respiration, temperatures, O2, saturations and finger sticks.
Performs EKGs, hearing and vision tests, Urine Dipsticks, Urine Pregnancy Tests and other CLIA waived tests when necessary.
Orders supplies and maintains records of purchased equipment and supplies.
Attends school functions and serves on school committees as needed.
Performs all other duties as required.
Mobile Van Driver (As Assigned):
Drive the mobile van to various locations as needed.
Maintain mileage, travel logs, and prepare reports as needed.
Maintain fuel levels and adhere to preventative maintenance schedule.
Maintain a clean and mechanically-sound vehicle and ensure clinic is fully operational.
Assist in coordination of basic vehicle maintenance and repairs.
Assist with delivery of patient care services and management, and navigation of participants on and off the mobile unit.
Performs equipment checks, pre-trip and post-trip inspections, and ensures overall cleanliness before, during and after visits.
Maintains supply inventory.
Other duties as required to support the Mobile Medical Vehicle's readiness to operate and to accept participant encounters.
Performs all other duties as required.
Requirements:
High School Diploma/GED required.
Three to five (3-5) years of experience as a Medical Assistant or Certified Nursing Assistant required.
Must have diploma from accredited Medical Assistant or Certified Nursing Assistant program.
Phlebotomy Certification from National Phlebotomy Association or National Healthcare Association preferred and current CPR certification from the American Heart Association required.
Bi-lingual English/Spanish strongly preferred.
All skills of Medical Assistant must be met.
Tractor/trailer/bus driving experience preferred or graduated from an acceptable truck driving training program and have 2 years of driving experience. Training is available for staff who are motivated, otherwise qualified, and interested in learning.
Excellent interpersonal skills, organizational skills, and written and verbal communications skills are essential.
Computer skills and data entry skills are required.
Familiarity with Microsoft, Internet and E-mail is necessary. Computer skills are required with the ability to learn Epic EMR and Outlook;
Ability to float to other departments or sites and perform all aspects of patient care.
Ability to prepare patients for examination by performing preliminary physical tests; taking blood pressure, weight and temperature; verifies patient information by interviewing patient; recording medical history; confirming purpose of visit.
