Mount Sinai Health System Clinical Pharmacy Manager; Ambulatory Care and Population Health; Full Time; Days in New York, New York

Job Description

The Department of Pharmacy is driving this charge within Mount Sinai. Our department includes 400+ employees with clinical, drug information, investigational drug services, infusion center, management, operations, pharmacy supply chain, specialty, and retail capabilities.

Members of the team will be responsible for medication management quality, cost, utilization and clinical stewardship activities for populations managed and at a program level.

Qualifications

Education:

? Doctor of Pharmacy Degree required

? ASHP-Accredited PGY1 Pharmacy Residency required

? ASHP-Accredited PGY2 Pharmacy Residency (Ambulatory Care) preferred

? Board certification (BCACP) or obtain BCACP within 2 years of hire, required. Depending on the specific patient

? population that the ambulatory care clinical pharmacy manager cares for, an alternative BPS specialty

? certification (BCGP, BCCP, BCTXP, BCPP, etc) may be approved by the Director.

Experience Requirements

? 3 years health system experience or equivalent, with clinical experience/training; Ambulatory practice setting

? preferred.

Licensing and Certification Requirements

? Name: Registered Pharmacist Issuing Agency: NYS Board of Pharmacy

? Continues to meet regulatory, educational, and licensure requirements throughout employment, as set forth by New York State regulations.

? Achieves annual requirement of 15 continuing education hours (ACPE approved).

? Must be certified (or certified within 6 months of hire) to practice Collaborative Drug Therapy Management

? (CDTM) by the New York State Education Department

? Immunization certification by the New York State Education Department, preferred within 1 year of hire

? American Heart Association BLS certification, required within 2 months of hire

Non-Bargaining Unit, 600 - HSO Pharmacy - MSH, Mount Sinai Hospital

Responsibilities

#1 Delivery of clinical pharmacy services with a focus on chronic disease management

• Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs (including Remote Patient Monitoring and Chronic Care Management)

• Serve as a clinical pharmacy resource for the health-system and its affiliated pharmacy services for the Clinically Integrated Network (CIN)

• Work collaboratively with clinical staff to implement and modify medication therapy of individual patients (including those with diabetes, hypertension, etc.)

• Implements decisions of and provides support for the Ambulatory Pharmacy & Therapeutics committee by preparing drug evaluations and assisting in formulary maintenance

Responsibility #2 Pharmacy Strategy and Innovation

• Identification, development, and implementation of new pharmacy strategies and innovation pilot projects

• across responsible clinics/areas.

• Provide education, training and work direction to clinic staff and practitioners as required.

Responsibility #3 Precepting

• Serve as a preceptor for the MSHS PGY1 and MSHP PGY2 ambulatory care pharmacy residency programs

• and students from colleges of pharmacy

• Provide scholarly activities and presentations at local and national meetings to advocate for and raise

• awareness of population health pharmacy roles and responsibilities

Responsibility #4 Management

• Develop and scale condition and medication management program, manage goals of pharmacy integration

• across MSHS, CIN employed or voluntary practices.

• Support financial, quality and utilization analytics and performance improvement evaluations.

• Serve as pharmacy representative on behalf of MSHP as required on organizational committees.

• Supervises pharmacy employees

• Responsible for/contributes to employee performance evaluations for assigned staff

• Provides off-hour coverage for the department

• Interviews candidates and supports in leading orientation activities

Compensation Statement

Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $159000 - $190565 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
