Job Summary and Qualifications

• The Manager of Case Management assesses, plans, facilitates, and advocates options and services to meet a patient’s health needs, or social needs that may impinge on their medical condition, through communication and available resources to promote quality cost-effective outcomes and regulatory compliance.

• The Manager of Case Management works collaboratively with the interdisciplinary staff internal and external to the organization. The Manager of Case Management participates in performance improvement and evaluation processes related for the case management team.

• The Manager of Case Management shares responsibility for the planning, direction, coordination, and control of activities concerned with case management and social services.

• Assists in the provision of resources and assists to remove barriers to successful operation of case management and social services.

What qualifications you will need:

Education/Licensure:

• Texas State RN license or compact license required.

• Bachelor’s degree in Nursing required.

Experience Required:

• Five (5) years RN experience in an acute care facility with three (3) years supervisory experience preferred.

Special Skills & Qualifications:

• Certification in Case Management preferred.

• Computer skills in word processing and spreadsheet applications preferred
