Description

Gastroenterology Registered Nurse Navigator - Norwalk, CT Full time 40 hours per week. Monday-Friday 8:30am-5:00pm.

Summary:

To create a nurse navigator for the Digestive Health Institute to support the Advanced GI program for Danbury and Norwalk GI practices. Role will be to keep patients in our system by navigating them to other sub-specialists and services with regard to advanced GI. This position will support the early detection and assessment of pancreatic cysts.

Responsibilities:

  1. Conducts case findings. Screens patients to identify support, education, discharge planning and referral. Comprehensive assessment of patient's physical, functional, psychosocial and financial status in cooperation with the health care team.

  2. Develops, implements, and evaluates the plan of care and support through an interdisciplinary process in conjunction with the patient and family, linking the patient to appropriate institutional or community resources.

  3. Procures services; serves as an advocate for patients and families with scarce resources. Facilitates referrals to state and local financial assistance agencies or programs.

  4. Functions as a liaison between the hospital, physicians, patient, and community resources to achieve maximum communication between all parties so that care is coordinated, and all needs of the patient are met.

  5. Identifies quality issues related to cancer diagnosis, treatment and follow-up, risk management issues, and reimbursement issues with prompt referral to appropriate committee/personnel for action or resolution.

  6. Assesses and evaluates care through interaction with patients and surveys, and implements quality improvement projects as needed.

  7. Serves as a representative of the hepatobiliary service and lead person for coordination of care for patients through the new early detection and evaluation of pancreatic cysts.

  8. Supervises the volunteers within the cancer program, assuring compliance with appropriate patient interaction and confidentiality of information.

  9. Participates as a member of the Hepatobiliary Comm.; achieves the organization's established expectations regarding customer service, teamwork, safety, and self-development.

  10. Fulfills all compliance responsibilities related to the position.

  13. Performs other duties as required.

Other Information:

CT RN License. Bachelor's Degree required. Oncology certification and prior experience a plus.

Manual: significant manual skills/motor coord & finger dexterity
