A Brief Overview

Home Health Aid

Day-To-Day:

You will visit 4-6 patients a day depending on location.

 A day in the life of a hospice home health aide involves providing compassionate care and support to patients. Here are some of the key tasks they perform daily:

 Personal Care

  Bathing and Hygiene: Assisting patients with bathing, grooming, and other personal hygiene tasks to ensure they feel clean and comfortable

  Dressing and Mobility: Helping patients get dressed and assisting with mobility, such as transferring from bed to chair

 Comfort and Dignity

  Skin Care: Keeping the patients skin clean and dry to prevent bed sores

  Emotional Support: Offering companionship and emotional support, which can be as simple as listening to the patients stories

 Household Tasks

  Light Housekeeping: Performing light housekeeping tasks like changing bed linens, doing laundry, and keeping the patients immediate area clean

 Monitoring and Reporting

  Health Monitoring: Observing and reporting any changes in the patients condition to the hospice nurse or other healthcare team members

 Family Support

  Education and Guidance: Providing guidance to family members on how to care for their loved one and offering emotional support during this challenging time

Hospice home health aides play a crucial role in maintaining the comfort and dignity of patients, while also providing much-needed support to their families. Their work is both challenging and deeply rewarding

Assessment and Collaboration:

Assessing patients and their families for physical, psychological, social, and spiritual needs.

Collaborating with patients, families, physicians, and the interdisciplinary group (IDG) to establish best care for the patient

Implementing interventions and evaluating care effectiveness.

Effective Communication:

Initiating and maintaining communication with attending physicians, the Medical Director, IDG members, Team Directors, Team Managers, and other relevant agencies.

Coordinating optimal care and resource utilization for patients and families.

Accurate Documentation:

Documenting patient information according to established protocols.

Maintaining up-to-date patient records to reflect baseline, current, and ongoing status, including any changes.

On-Call and Admissions:

Participating in on-call duties and admissions assignments as directed.

Patient Education:

Providing education to patients and caregivers based on their specific knowledge needs and learning assessments.

Addressing patient and family conditions with empathy and expertise.

Coordination and Problem Resolution:

Coordinating care with the IDG.

Informing the Team Director and Team Manager of any unusual or potentially problematic patient/family issues.

Professional Development:

Participating in orientation and ongoing education programs.

Maintaining current professional licensure.

Compliance and Protocols:

Demonstrating familiarity with agency and state policies and protocols related to hospice and nursing services.

Ensuring compliance with internal process requirements and state/federal regulations.

Collaborative Meetings:

Participating in IDG meetings, facility patient conferences, and other patient/family meetings as coordinated with the IDG and/or Team Director.

Additional Responsibilities:

Fulfilling any other duties as assigned.

Skills and Requirements

Education Qualifications

  1. Certified CNA (Certified Hospice and Palliative 2. Nursing Assistant - CHPNA) OR certified Home Health Aid (HHA course and cert)

  2. BLS - Basic Life Support

Experience

Hospice, skilled nursing facility or an assistant living experience for 6 months to a year EMR Care Point (homecare/homebase/point care) (BIG PLUS) null
