$240,000 - $280,000 for full-time status, based on experience

JOB DESCRIPTION

Community Health Center of the North Country is a Federally Qualified Health Center (FQHC) located in Upstate New York. CHCNC provides high quality healthcare services to the surrounding communities and offers a sliding fee scale. With four locations, CHCNC is a growing healthcare organization that offers primary, acute, and pediatric medical care, dental services, physical therapy, and behavioral health services to the local communities. The sliding fee scale ensures that uninsured or underinsured patients receive the same quality of care as those individuals who are commercially insured. Patients needing medical care can benefit from the affordable care options provided by CHCNC, ensuring they are not denied care based on their financial situation. We are seeking a highly skilled and compassionate Family Medicine / Internal Medicine Physician to join our team. The Physician shall provide high-quality medical care by examining, treating, and educating patients on the prevention and promotion of healthy habits. The Physician will be responsible for providing comprehensive medical care to patients across a wide range of ages, diagnosing illnesses, managing chronic conditions, and referring patients to specialists when necessary, while building long-term patient relationships and addressing overall health needs. The Physician will work collaboratively with other healthcare professionals to ensure the highest quality of care for our patients.

Primary Duties and Responsibilities

  • Conduct comprehensive medical evaluations and assessments of patients

  • Daily patient volume: 16/daily for 8-hour shifts; 20/daily for 10-hr. shifts.

  • Develop and implement treatment plans for acute and chronic illnesses

  • Order and interpret diagnostic tests and imaging studies

  • Prescribe medications and other treatments as needed

  • Provide patient education and counseling on disease prevention and healthy lifestyle choices

  • Collaborate with other healthcare professionals to coordinate patient care

  • Maintain accurate and up-to-date medical records

  • Responsible for supervising the quality assurance program and reporting the program's activities to the Board of Directors.

Requirements

  • Medical degree from an accredited institution

    • Board Certificated or Board Eligible in Family Medicine or Internal Medicine

    • Active and unrestricted NYS Medical License

    • Active DEA License

    • Current BLS Certification

    • Strong clinical skills and knowledge of current medical practices and procedures

    • Exhibits a high level of professionalism and confidentiality in dealing with issues.

    • Ability to communicate effectively

    • Ability to work collaboratively with other healthcare professionals

    • A strong commitment to providing compassionate and patient-centered care

    • Provide appropriate supervision and/or serve as a collaborating physician for Advanced Practice Providers
