The Community Outreach Rep Sr in the Optum at Home program is responsible for sourcing, developing, and maintaining relationships with external program stakeholders, including key health systems, hospitals, PCPs and IPAs, for the purpose of recurring operational and clinical collaboration, which will drive improved Quality metrics, reduce utilization, and increase engagement. Through provider and community marketing and outreach efforts, the Community Outreach Rep Sr will have a direct impact on membership growth and retention.

The Community Outreach Rep Sr will work in alignment with the Market Leadership team in identifying appropriate target relationships, clinical collaboration opportunities and engagement drivers. The Community Outreach Rep Sr will keep market leadership informed of status of relationships, opportunities for growth and any areas of concern.

The Community Outreach Rep Sr will maintain ownership and responsibility for all aspects of the relationship to include but not limited to: serving as primary point of contact, researching and communication of resolution regarding any concerns, scheduling, preparing materials for and leading recurring collaboration meetings to address communication, operations and clinical/quality alignment with assigned health care entities.

Primary Responsibilities:

  • Account owner/driver of relations for all assigned health entities – communications, strategy and growth

  • Alignment with MLT, as it relates to collaborative relations and metric improvement of assigned entities

  • Collect and analyze marketing, health, and community-related data to drive relationship priority identification and account focus

  • Create and execute plan for development and growth of relationship with assigned entities (communication, operations, data exchange, clinical collaboration, quality gaps/metric review, review of MLR/Utilization)

  • Coordinate and lead recurring operational/clinical collaboration meetings with assigned accounts

  • Define and deploy strategies to engage external clinical stakeholders in collaborative partnerships to drive program enrollment and retention

  • Internal/External Relationships

  • Perform regular virtual and in-person meetings with key health systems, hospitals, PCPs and IPAs

  • Establish collaborative clinical meetings between care management teams to foster care coordination, utilize available resources/benefits to enhance the delivery of care to patients and to enhance endorsement of the OAH in home program

  • Coordination with key health systems, hospitals, PCPs and IPAs to develop engagement priority lists of patients

  • Collaboration with central outreach team to drive targeted engagement efforts

  • Organize, schedule and prepare materials and status updates for JOCs

  • Present overview of clinical model, value drivers and clinical outcomes of the Optum at Home in home program

  • Develop strategy in concert with Market Executive Director to promote Optum at Home with patients with key health systems, hospitals, PCPs and IPAs

  • Drive collaboration and identify joint strategies to enhance affordability initiatives with key health systems, hospitals, PCPs and IPAs to avoid unnecessary admissions and emergency visits, such as promotion of urgent care services, use of co-branded letters endorsing the Optum at Home in home program and coordination of end-of-life planning

Required Qualifications:

  • 5+ years of experience in provider engagement, provider relations, provider network or key account management within the healthcare industry

  • Proven ability to skillfully present high-quality, impactful PowerPoint presentations

  • Proven ability to successfully work in a cross functional environment

  • Proven ability to engage in strategic and planning discussions at multiple levels with an external organization including, but not limited to, CEO, President, CMO, CNO and Directors

  • Proven success meeting or exceeding identified targets or goals

  • Proven success with time management and organizational skills with minimal need for supervision

  • Willing to work flexible or extended hours to accomplish business goals

  • Access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy

Preferred Qualifications:

  • Experience in population health, extensive knowledge of value-based contract models, HEDIS/STARS/CAHPS, MLR optimization

  • Experience influencing others at many organizational levels, both internally and externally

  • Ability to lift up to 40 pounds

  • Valid Driver’s License with the ability to travel up to 75% of the time between care settings

Technical Knowledge & Skills:

  • Impeccable verbal and written communication skills

  • Professional demeanor and high level of emotional intelligence

  • Proficient in Microsoft Excel, Word and PowerPoint, CRM use (such as Sales Force)

  • Solid organizational skills and multitasking abilities will be keys to success

  • Data analysis capabilities including navigating management reports and using data to monitor performance and identify trends
