PACT Care Coordinator - $1,000 Sign-On Bonus!
Provides clinical case management services to designated priority clients in the Program for Assertive Community Treatment (PACT). The goal of the position is to help clients maintain stable community living, increase independence, and enhance quality of life in the community.
Compensation: $23.38- $31.25 per hour depending on experience and qualifications.

QUALIFICATIONS:
- Bachelor's degree in psychology, social work or closely related field.
- Previous experience working as a member of a multidisciplinary treatment team and working collaboratively with other service providers.
- Strong organizational skills to include use of Electronic Health Record software or equivalent for scheduling, time management and all clinical documentation.
- Have a valid driver's license and auto insurance.
