Hospital At Home is a very dynamic model of care, where appropriate patients transfer to the virtual unit to complete their hospitalization. This program runs like a regular inpatient unit, it just happens to be set up in the patient's home. The program provides medical equipment, meals, and diagnostics in the home setting. The Physical Therapist will provide the 'hands on' care and be part of a team of clinicians which includes Command Center RNs, MDs, plus Field Therapists. This role is very dynamic as, the Physical Therapist will work closely with their virtual colleagues throughout the day to provide clinical care for our patients. Hospital at Home is a model of care where patients who meet inpatient criteria are clinically managed in their home. Data shows that patients cared for in a Hospital at Home model have better outcomes, higher patient/family satisfaction, fewer complications, and shorter lengths of stay. Staff who care for these patients virtually and in the home also report higher job satisfaction. The Physical Therapist is an integral part of providing the best care possible for a subset of the patient population. The acuity of patients in our program are the same as a standard medical floor. And, while this care model isn't appropriate for everyone, it is appropriate for those patients who meet specific, established criteria, and are deemed safe and appropriate to be hospitalized in their home versus the traditional setting. Strong candidates will have excellent clinical and communication skills, be comfortable with technology, and a willingness to flex as the day progresses. You will be joining a team of very supportive clinicians. The Physical Therapist, as directed by a health care practitioner, performs PT evaluations, establishes treatment plans with goals, provides treatment, delegates specific treatments, determines appropriate equipment needs and performs discharge activities. Required Qualifications: Bachelor's Degree in Physical Therapy or Master's Degree in Physical Therapy or D.P.T. in Physical Therapy Washington Physical Therapist License upon hire National Provider BLS - American Heart Association upon hire Preferred Qualifications: 3 months of experience in either acute, orthopedics, neuro or lymphedema Why Join Providence?
