CHI Health would love to add a Clinic RN (Nurse) Navigator to our Urology Clinic located at our CUMC-Bergan campus off of 77th & Mercy!

Hours: Monday-Friday 8a-5p, No weekends, No holidays & No call!

Our full-service network provides a variety of Primary Care & Priority Care Services, including Family, Internal, Geriatric, and Pediatric Medicine, in addition to several specialties to deliver custom care based on the unique needs of our patients.

Job Summary / Purpose

Provide comprehensive care coordination for patients, families, and caregivers by serving as the designated contact, facilitating interactions with the clinical care team, and providing seamless navigation through the health system.

Essential Key Job Responsibilities

  • Collaborate with providers to develop and implement standards of care for select patient populations.

  • Function as an interdisciplinary team liaison for patients by assessing, securing, and coordinating appropriate services through the continuum of care.

  • Educate patients about diseases, treatments, side effects, side effect symptom management and how to access reliable sources of information.

  • Provide clinical resources for treatment or testing that may not be available locally.

  • Assess patients for psychosocial distress and/or tangible barriers to care such as finances, housing, transportation, childcare, and prescription costs. Refer patients to the appropriate resources, such as a medical social worker or counselor.

  • Empower patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness or other factors.

  • Promote awareness of appropriate clinical trials in consultations with the provider.

  • Other duties as assigned by management.

Qualifications

Minimum Qualifications:

  • Graduate of an accredited school of nursing

  • Active, unencumbered Registered Nurse (RN) license from the State licensing board (must be licensed in the State in which patient care is provided)

  • Basic Life Support (BLS) from the American Heart Association required upon hire

  • 1 year related experience in a clinic, specialty clinic or acute hospital setting

  • Experience with oncology strongly preferred

Pay Range

$27.04 - $39.21 /hour
