The role you’ll contribute:

The Clinical Manager is a Registered Nurse or Physical Therapist who coordinates and supervises the overall clinical care such as scheduling, coordination and supervision of the home health interdisciplinary team. The interdisciplinary team, in consultation with the patient/family and physician(s), collectively assesses, coordinates, develops and implements a plan of care (POC) that is directed towards teaching the patients/families/caregivers how to manage the illness at home, assist the patient and family through the rehabilitative process, and teaches them how to return to the highest level of functioning possible to maintain independence in their own environment. This position may be designated as the Director of Nursing and/or Alternate Administrator for regulatory purposes.

This position supervises and directs the activities of various levels of assigned nursing and ancillary personnel utilizing both professional and supervisory discretion and independent judgment, while practicing within the State, facility and regulatory guidelines.

  • Responsible for effectively planning, coordinating, and managing all clinical activities and programs for home health in accordance with regulatory guidelines and professional standards.

  • Provides clinical oversight and scheduling coordination as required for optimal clinical functionality.

  • Maintains and interprets operational indicators to detect census/volume changes that can impact staffing needs, productivity, revenue and expenses, quality indicators, patient satisfaction, clinical and financial scorecards.

  • Improves outcomes through planned and continuous improvement initiatives to ensure quality patient care is delivered in a cost-effective manner.

  • Establishes patient specific POCs with clinicians, assuring completeness of orders and goals, and congruency with OASIS assessment.

  • Establishes correct ICD-10 codes, in collaboration with clinician and appropriate coding resource.

  • Collaborate with Central Billing Office staff for medical review and billing needs and questions.

  • Assures medical recertification processes occur in appropriate time frames.

  • Assists with internal orders, DNB and claim edit work queues as requested.

The expertise and experiences you’ll need to succeed:

Minimum qualifications :

  • Associate’s degree in nursing or other healthcare related field.

  • Three (3 ) years of experience in home care or related healthcare field.

  • Clear/active Registered Nurse or Physical Therapy license issued by the state in which agency is located.

  • Maintain current, valid Driver’s License in good standing with a safe driving record and a reliable vehicle.

  • Maintain valid personal automobile insurance.

  • Basic Life Support certification.

Supervisory Responsibilities:

  • Supervises staff providing direct patient care such as home health aide, RN, LPN, PT, PTA, OT, COTA, SLP, MSW. May also supervise other office staff who support the delivery of care as per organization chart.

Preferred Qualifications:

  • At least one (1) year of experience in clinical supervisor or quality review activities.

  • Bachelor’s degree in nursing or other healthcare related field.
