Responsibilities

To develop an activity program whereby patients/residents have the opportunity to function at their highest physical, social, cognitive and/or emotional level. Plan monthly activity programs in advance, develops activity monthly calendar to be made available to patients/residents. The activity program shall include games, mental quizzes or puzzles, music, divisional and restorative activities, etc., geared to individual patient/residents interests. Responsible for assessing, identifying, and developing patient/resident activity needs/plans. Reviews and maintains proper documentation in the patient/resident medical record. Participates as a member of the multidisciplinary care conference team. Assists with evaluation and training of activity assistants to assure the competency and skills are maintained. Provides competent patient care as needed.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

  1. Certification as an Activities Coordinator or Activities Professional (APC) preferred.

  2. Must have the ability to work and communicate well as a part of the Interdisciplinary Care Team, both as it pertains to the care plans and M.D.S. completion.

  3. Must have knowledge of all current State and Federal Regulations as they pertain to Social Services and Activities.
