In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. Job Summary:
Under the direction of the Reproductive Health Program Manager, the health advocate will educate patients on the importance of family planning services to increase reproductive health outcomes related to cancer prevention. Team member will focus on breast and cervical cancer prevention through assisting with mammograms, pap smear access and HPV vaccination. Health advocate will develop, implement, and evaluate comprehensive patient plans to ensure that patients receive appropriate overall medical care. Health Advocate will work as an integral part of interdisciplinary team, ensuring excellence in patient care. Reproductive health outcomes include cancer screening for breast and cervical cancer through proper screenings, education and vaccination. Enabling and promoting informed decision making on behalf of Finger Lakes Community Health in all coverage areas, assisting in the recruitment of new patients.
This position will cover Yates and Steuben Counties
Duties/Responsibilities:
Manages patient roster for cancer screening around breast and cervical cancer
Outreach and education to patients who are due for preventative cancer screenings
Advocacy to patients about importance of regular cancer screening and preventative measures
Focuses on internal health advocacy for patients of Finger Lakes Community Health
Leads partnership programs to assist patients in accessing mammograms
Represents Finger Lakes Community Health at community agency/network meetings
Assist in scheduling reproductive health services
Notes and reports on current trends and statistics in order to share information with patients, staff, program participants and colleagues.
Participates in appropriate community events
Engages in campaigns related to reproductive health, in the Finger Lakes Community Health coverage area
Researches and approaches appropriate agencies to encourage expansion of FLCH services and establish new relationships
Ensures that promotional items are available and in areas accessible to patients and agencies
Prepares and complete monthly reports in compliance with NYS Family Planning Program regulations
Enters client information correctly into Electronic Health Record system (EHR)
Sees patients in office for Reproductive Health Education
Performs additional duties as assigned by supervisor
Required Skills/Abilities:
Possesses excellent interpersonal, communications, public speaking, and presentation skills
Possesses intermediate computer skills
Demonstrates effective working relationships, utilizing the ability to work individually or as part of a team
Possesses the ability to flex schedule to include evenings and weekends
Ability to communicate in Spanish and English preferred
Education and Experience:
1-3 years experience in counseling, education, youth work or social work strongly preferred
Possesses valid New York State drivers license in compliance with FLCH drivers policy
Current New York State automobile insurance in compliance with FLCH underwriting agreement
Physical Requirements:
Must be able to hear and communicate with clients and staff via phone, web media, and in person.
Must be able to lift up to 50 lbs
Must be able to sit for extended periods of time
