Description:

LIFE at Lourdes, a member of Trinity Health PACE, is seeking a certified Home Health Aide, to work every other weekend, for its location in Pennsauken, NJ. Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.

Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

Home Health Aide

Status : Part Tim

S hift : Flexable Schedule - Weekend and Holiday rotation

Position Purpose:

The Home Health Aide is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants.

What you will do:

  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin Home Health Aide integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.

  • Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards.

  • Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.

  • Attend staff meetings and participant care planning on a regular basis.

  • Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PO vehicles as needed.

  • Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participant’s expressed wishes.

  • Prepare and submit timely written reports as required.

Minimum Qualifications:

  • High school diploma or equivalent required. Active Home Health Aide certification in the state PO is located required. Certified Nurse Assistant Certification preferred

  • Active CPR Certification required

  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
