Retirement Savings Plan Elwyn is currently seeking a Family Resource Coordinator/Social Worker to provide ancillary social work supports and services to families receiving special education and behavioral health services within Elwyn's Children's Services division. Rooted in the enduring commitment to supporting the service triangle, the Family Resource Coordinator offers individualized and group support, resource coordination, community linkages, and guidance to parents and caregivers whose children are served by programs within Children's Services. The Family Resource Coordinator develops, maintains, and updates resources and supports families of children with disabilities in navigating available resources throughout various internal and community-based service systems and parent-to-parent networks to champion their child's overall success and meaningful progress. This position applies an evidence-based social work framework, performs caregiver/family education on special education systems, participates in child find activities, facilitates parent-to-parent engagement and support, promotes family engagement, and recognizes parents as valuable partners in the process of holistic treatment and education of children. DUTIES AND RESPONSIBILITIES: Provide supervision of assigned staff in the areas including, but not limited to, recruitment, selection, leadership, coaching, training, performance management, promotion, discipline, and termination Employ a high-quality, client-centered, customer service approach including, but not limited to, active listening, interview and assessment, identification of needs, resource coordination, and resolution including the provision of any applicable community supports as a complement to the services already received through Elwyn, by actively promoting community engagement and recreation, respite and home supports, basic needs and/or emergency supports to all families on a regular basis Facilitate key linkages to community resources, facilitate referrals, and coordinate connection to services via phone, video conference, or in-person meeting as needed to meet the ongoing needs of the family by engaging in follow-up calls and visits as necessary to ensure appropriate follow through and coordination of services to the family; initiate contact with the families, as required, to ensure access, navigation, and initiation of the service; provide a high quality, highly responsive customer service experiences for all parents and families seeking assistance Develop, maintain, and update the Elwyn Family Resource database with relevant resources including, but not limited to, camps, before/after care, school programs, autism resources, behavioral health/mental health, Medicaid/Medicare, medica insurance, Disability/Supplemental Security Income (SSI), housing support, homeless support, food/meals, child care centers, Child Abuse Hotline, suicide/self-harm, cultural resources, aquatics and recreation, wellness, respite care, Department of Human Services (DHS), Temporary Assistance for Needy Families (TANF)/ Supplemental Nutrition Assistance Program (SNAP)/county assistance, faith-based organizations, language/English as a Second Language (ESL) services, literacy, hospitals and medical centers, library resources, Lesbian/Gay/Bisexual/Transgender/Queer-Questioning/Intersex/Asexual (LGBTQIA+), parenting classes, and parent support that are centrally-located and easily accessible; maintain, update, and contribute to the active directory of parent resources and supervise and collaborate with interns on the identification of regional community resources to support and supplement the services received through the special educational system and Elwyn Conduct regular in-reach activities to parents and caregivers including participating in the development and implementation of quarterly newsletters, parent cafes, social events, family forums, information sessions, holiday celebrations, and welcome events to galvanize, coordinate, and connect new parents with programs, resources, and veteran parents for moral support, guidance, encouragement, and partnership; assist families in jeopardy of involuntary discharge or voluntary withdraw from services, employ conflict resolution as needed, and assist families in the decision-making process, employ a model of positive parent support Employ foundational principles of cultural humility, understanding, and support for families for whom English is a second language including, but not limited to, utilizing language line support to communicate with families, and/or translation services, as appropriate and employ a broad-based cultural approach within the larger context of accepting differences including, but not limited to, socio-economic, religious or political beliefs, gender and sexual identity, country or origin, and neighborhood demographics to ensure broad inclusion and access to relevant services and supports Promote and encourage parent and caregiver engagement in activities that promote inclusion, student achievement, and pride in membership including, but not limited to, community meetings, Transition-to-Kindergarten activities, Family First Trainings, Philadelphia Interagency Coordinating Council (PICC) activities/meetings, Parent Education and Advocacy Leadership (PEAL) Center activities, Parent Action Network (PAN), participation and coordination of Family Fun Day events, Elwyn Day, the Elwyn Ball, community/neighborhood fairs, and table outreach events by making families aware of these resources and activities Receive referrals from the respective program for children requiring additional, supplemental supports over and above what the program offers, conduct systems assessments, and provide generalist social work services to families in need of extended follow up, home visitation, and/or who are demonstrating a higher need for support to encourage child and family success; initiate additional community referrals, as needed, and share information with the program(s) in which the child is currently enrolled by employing a motivational interviewing, strengths-based, and systems-oriented approach to intervention; initiate community referrals, as appropriate, and complete session notes Maintain a working knowledge of Preschool Early Intervention, school-aged special education, and behavioral health regulations including, but not limited to, Procedural Safeguards, Free and Appropriate Public Education (FAPE), Least Restrictive Environment (LRE) regulations, Family Educat
