Requires sound judgment in making decisions and broad knowledge of departmental and hospital policies and procedures. Essential Functions: Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson Engage patients referred or identified for services and other as needed via telephone, email, nd electronic health record messages. Provide community outreach with attendance and participation in local community events to bring attention to the language or disability-focused services available at Jefferson Einstein Philadelphia Provide culturally sensitive services to patients from diverse racial, cultural, and socioeconomic backgrounds. Act as single point of contact for a panel of patients and their family members needing to access language-concordant healthcare services within Einstein. Assist patients to navigate our healthcare system, including, but not limited to, helping patients schedule specialist appointments or follow-up appointments with language proficient providers; arranging appointments with disability-focused providers, coordinate appointments for radiology or follow-up studies; request medical interpreters if language-proficient providers or services not available; prescription refill requests; and accompany patients to appointments as needed. Provide follow-up encounters with patients to ensure patientsrsquo; needs have been met Research and connect patients to community resources related to their needs, including food, housing, transportation, legal services, and other identified needs. Performs other job-related activities as required.
