Provide community outreach with attendance and participation in local community events to bring attention to the language or disability-focused services available at Jefferson Einstein Philadelphia Provide culturally sensitive services to patients from diverse racial, cultural, and socioeconomic backgrounds. Act as single point of contact for a panel of patients and their family members needing to access language-concordant healthcare services within Einstein. Assist patients to navigate our healthcare system, including, but not limited to, helping patients schedule specialist appointments or follow-up appointments with language proficient providers; arranging appointments with disability-focused providers, coordinate appointments for radiology or follow-up studies; request medical interpreters if language-proficient providers or services not available; prescription refill requests; and accompany patients to appointments as needed. Provide follow-up encounters with patients to ensure patientsrsquo; needs have been met Research and connect patients to community resources related to their needs, including food, housing, transportation, legal services, and other identified needs.
