Join the Deer Meadows team as a Physical Therapy Assistant (PTA) and provide top-tier home care services. Leverage your therapy skills to assess patient needs, follow care plans, and administer treatments. Collaborate with physicians and community health professionals to deliver comprehensive care.

Responsibilities:

  1. Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.
  2. Treats patient, directs and aids patients in active and passive therapeutic exercises; utilizes physical agents and observes and reports to the physical therapist and instructs patients in care and use of equipment.
  3. Maintains necessary records, clinical notes and conference notes.
  4. Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.
  5. Performs other duties as assigned.

Qualifications:

  • Possesses a degree from at least an associates program in physical therapy approved by an accredited organization.
  • Licensed to practice as physical therapy assistant within the state, if applicable.
  • Two years of appropriate experience as a physical therapist assistant. Community/home health experience is preferred.
  • Possesses and maintains current CPR certification.
  • Valid driver's license and reliable transportation
