THE RUNDOWN:

The Population Health Department works to enhance patient health, improve the health of populations, reduce the cost of care, and create efficiencies in CHC/SEKs system of care. This is achieved through the identification, management, and outreach to groups (such as children or diabetics), as well as through projects to improve workflows to meet quality goals, measures, and standards of care for the patient population in general.

Population Health staff are knowledgeable about CHC/SEK care systems and community resources, adaptive to patient needs and situations, and work to reduce barriers to care. They strive to understand, motivate, and help our patients by building rapport, eliminating barriers, and collaborating with other members of the care team. The Health Promotion Coordinator should be motivated and passionate about improving health outcomes.

This position is responsible for supervising the Health Promotion Specialists (HPS), assuring that established procedures and workflows/processes are followed, evaluating staff for training gaps, coordinating training for staff and organization as necessary on quality measures, and evaluating data-based statistics to determine staff focus. The coordinator serves as a resource for all of their routine functions and provides daily support to the Health Promotion Specialists and Population Health leadership.

  • Provide direct supervision of the Health Promotion Specialists (HPS) including hiring, training, performance evaluation and discipline when needed. As well as routine monitoring of time/attendance, focus, and productivity.
  • Serve as a resource to HPS by giving guidance on decision-making, providing information, and maintaining daily operations and workflows. Ensure that established policies, procedures, and workflows/processes are being followed.
  • Gather and analyze data related to quality metrics from multiple data sources, including electronic medical records, payer portals, the Accountable Care Organization (ACO) platform, and Azara. This data will be used to identify trends and to inform decision-making and program development in support of payer performance measures, UDS, and more. Communicate relevant data with HPS as directed by leadership and/or payers.
  • Facilitate a weekly huddle for HPS to share updates and performance-improvement needs, communicate goals, assign special projects, evaluate needs, etc.
  • Assist in setting individual and team goals/performance targets related to Population Health initiatives. Conduct ongoing reviews to analyze progress and ensure departmental goals and outcomes are met.
  • Monitor Iris Healthcare referrals in the electronic health record to ensure completion of referrals and that advance care planning documents are received.
  • Assist HPS in downloading and organizing recurring Excel reports, such as patient no-show lists, upcoming wellness visits due for health risk assessments, etc.
  • Communicate pertinent information regarding HPS and their initiatives to Population Health and/or other organizational leadership.
  • Conduct routine reviews of staff workloads, productivity, and documentation to improve processes or provide new insights into performance and opportunities.
  • Perform periodic quality assurance audits by reviewing HPS documentation in patient outreach platforms to identify education/training opportunities for HPS.
  • Create and monitor internal workflows, resource guides, and other documents for correctness and relevance, and provide updates as needed.
  • Work directly with HPS to cultivate a high-performing team through ongoing education, training, and coaching to continuously improve expertise and efficacy.
  • Collaborate with a multidisciplinary team to develop innovative ways to engage patients, implement strategies to help patients complete gaps in their healthcare, and monitor effectiveness through performance indicators.
  • Attend meetings with payers that we participate in Value Based Care contracts with (either in person or virtually) monthly or on an as needed basis.
