Description:

POSITION PURPOSE:

  • The PATH (People Assisting The Homeless) Outreach Case Manager is responsible for providing comprehensive support to individuals experiencing homelessness.

  • This role involves outreach, case management, and coordination of services to help clients achieve stability and improve their quality of life.

  • Will work directly in the community to connect with individuals, assess their needs, and provide tailored support and resources.

WHAT YOU WILL DO:

Outreach and Engagement:

  • Conduct outreach activities in designated areas to identify and engage individuals experiencing homelessness.

  • Build and maintain relationships with clients to foster trust and encourage participation in services.

  • Provide immediate support and resources to individuals in crisis situations.

Case Management:

  • Develop individualized case plans based on client needs, goals, and preferences.

  • Assist clients in accessing resources such as emergency shelter, housing, healthcare, and social services.

  • Monitor client progress and adjust case plans as needed to ensure effective support and goal achievement.

Resource Coordination:

  • Collaborate with community organizations, service providers, and government agencies to coordinate services and referrals.

Documentation and Reporting:

  • Maintain accurate and confidential records of client interactions, case notes, and service outcomes.

Community Engagement:

  • Participate in community meetings, networks, and coalitions to promote awareness of homelessness issues and advocate for client needs.

  • Collaborate with other service providers to enhance the effectiveness of the homeless service delivery system.

MINIMUM QUALIFICATIONS:

  • Master’s Degree in Social Services with 1 year of social service experience OR

  • Bachelor’s Degree in Social Services with 3 years of social service experience OR

  • Associate degree in Social Services with 4 years of social service experience OR

  • High-school diploma/GED and 8 years of social service experience

  • Act 33/34/73 clearances

  • Valid Driver License

  • Ability to drive an organization vehicle

  • Pre-Employment Drug Screen

  • Strong interpersonal and communication skills, with the ability to build rapport and engage with individuals from diverse backgrounds.

  • Physical Requirement: Ability to conduct outreach activities in various environments.

POSITION HIGHIGHTS/BENEFITS

  • WORK SCHEDULE: This is a Non-exempt (hourly) position. Monday through Friday 8:00 am until 4:00 pm with flexibility to meet consumers’ needs. Nontraditional hours (evenings, weekends, and holidays) may be required.
