Applicants will be required to work 1pm to 10pm CST. If you have telephonic experience, working in a high volume Inbound and Outbound setting, triage experience, long with experience in cardiac/pulmonary, oncology, med/surg, GI, or BH with a Compact Nursing licensure and willing to be licensed in every state, this position is for you!! As an AccoladeCare Registered Nurse, you will be a vital member of the AccoladeCare team, playing a significant role in managing acute and chronic conditions. Creating lasting relationships is the foundation from which our AccoladeCare team is derived. Accolade Personalized Healthcare has a network of "virtual first" primary care providers (vPCP). Our AccoladeCare team will support our patients in "virtual first" primary care or local primary care delivery when appropriate or requested. Our goal is to be proactive by getting to know our patients personally, which will allow us to support them at a holistic human level. A day in the life... OPERATIONAL: * Leverage technology to proactively outreach to patients with complex, chronic medical conditions at a set frequency to monitor for self-management, reinforce disease management and identify the need for earlyintervention * Serves as the primary contact for patients in need of clinical support. * Identify health conditions needing urgent intervention, determine next best steps, and escalate patient needs when appropriate * Correspond with patients in whatever method is most suitable for them (phone, email, in-app) * Continuously educate patients to understand their treatment plans and when to contact their provider * Evaluate acuity to prioritize patient needs for acute and high-risk patients alongside Health Coach within the AccoladeCare team * Accompany and collaborate with critical external stakeholders and care team members during an acute or high-risk patient's transition of care * Guide patients to resources to educate them on their specific health conditions * Assure all acute or high-risk patients have an accurate and current care plan and ensure the AccoladeCare team is executing them * Ability to identify key trigger points to smoothly transition patients back to vPCP after referral/s or acute healthcare needs * Be available when patients need us * Follow-up and do the leg work when appropriate * Help patients to address barriers to care * Learn about the lives of patients allowing us to customize the support we provide to them and their families * Provide options to care and support patients when making treatment decisions * Educate patients to be effective/efficient consumers of healthcare by preparing them for visits, tests, and procedures * Educate and empower patients, so they confidently navigate the healthcare system * Analyze the impact of patient engagements to provide meaningful data back to patients * Any other duties needed to help drive to our Vision, fulfill our Mission, and abide by our Organization's Values What we are looking for... * Must have a compact state Registered Nurse (RN) license with a minimum of 4 years Required. Hiring Range $83280-$97160 Annual Actual compensation packages within that range are based on a wide array of factors unique to each candidate, including but not limited to skill set, years and depth of experience, certifications, and specific location.
