Honor Community Health Clinical Outcome Lead in Pontiac, Michigan

Position Description

Reporting to the Director of Health Center Operations, the full-time, hybrid position Clinical Outcome Lead is an essential member of a high-performing team providing support and training to staff. The clinical outcome Lead is responsible for the daily vaccine operation at all Honor sites, training and implementing new Honor clinical procedures, policies, and workflow. Will oversee vaccine services at all Honor sites, train new Medical Assistants, and retrain existing Medical Assistants. Clinical Outcome Lead will collaborate, lead assigned projects within the center, and provide ongoing support of services. The Clinical Outcome Lead also maintains excellent patient satisfaction, clinical outcomes, and Honors site productivity.

ESSENTIAL FUNCTIONS:

Clinical and Office Operations

  • Works at the direction of the Director of Health Center Operations to develop and implement best practices for clinic operations.

  • Works with the Director of Health Center Operation and site leaders to assess staff vaccine accuracy to ensure adequate and efficient workflow and address areas of concern.

  • Works with the Health Center Director to develop, implement and enforce training and re-training for Mas.  Collaborate with site supervisor in the training with all New Mas PCTs and retrain them with clinic workflow, procedures, and policies.

  • Collaborating with the Director of Health Center Operation and Clinical Quality manager, leads immunization-related quality improvement efforts per applicable standards.

  • Works with clinical sites to ensure vaccinations are ordered and inventoried appropriately, utilizing AccuVax and Accushelf.

  • Provide clinical support for AccuVax and Accushelf systems, including running reports, managing and supporting loading inventory, and monitoring vaccine stock.

  • Leads proper vaccine inventory at all Honor Sites.

  • Assist vaccine coordinators and site leaders with Oakland Health Department audits.

  • Served as the support for site leaders and vaccine coordinators with ordering vaccines.

  • Assist in developing strategic and management plans for the practice to enhance business growth.

  • Supports, upholds, and implements established policies, procedures, objectives, quality improvement, safety, environmental and infection control.

  • Collaborates with other practice managers, the Director of Health Center Operation, and the Chief Operating Officer to consistently uphold policies and procedures across all sites.

  • Apply evidence-based healthcare practices and nursing standards to all work as applicable.

  • Provide excellent customer service to all patients, staff, and community partners. 

Patient-Centered Care:

  • Works with the Director of Health Center Operation to ensure that medical personnel are trained to provide high-quality and efficient care to our patients to achieve optimum comfort and care.

  • Supports and ensures all MAs follow proper workflow to assist healthcare clinicians in examinations, procedures, and treatments.

  • Performs clinical and administrative duties as needed to assist staff during personnel shortages.

  • Upholds and ensures that all MAs are trained to maintain accurate and up-to-date records, including patient information, medical histories, and treatment plans.

  • Ensures and confirms all MAs protect patient confidentiality and adherence to privacy regulations.

  • Demonstrates and trains MAs for adherence to Honor Community Health policies and procedures, HRSA Regulations, Patient-Centered Medical Home standards, and local, state, and federal regulations.

  • Works with the Director of Health Center Operation to ensure competency training, assessments, and required record keeping in accordance with patient satisfaction, safety, and quality of care improvement, including, but not limited to:

  • Clinical Laboratory Improvement Amendment (CLIA) Criteria

  • 3rd party Laboratories in accordance with their requirements

  • The Occupational Safety and Health Administration (OSHA) Requirements

  • The Michigan Occupational Safety and Health Administration (MIOSHA) Regulations

  • Vaccine Management, Storage, and Handling, including-

  • The Vaccines for Children (VFC) Coordinator and Program Requirements

  • The Michigan Care Improvement Registry (MCIR) System Requirements

  •   Annual Education and Competencies

  • Works with the Director of Health Center Operation to facilitate performance improvement activities when required.

     

Training and Mentorship:

  • Oversees the smooth functioning of vaccine procedures in the clinic by working with the Director of Health Center Operation and Site Leaders to conduct performance evaluations and arrange training.

  • Works with the site clinician(s) to ensure medical personnel provide high-quality and efficient care to achieve optimum patient-centered care.

  • Coordinates ongoing work activities for continuous quality improvement, ensures completion of competency assessments, and facilitates performance improvement activities, including care improvement initiatives, patient satisfaction, safety activities, etc.

  •  Works with the Director of Health Center Operation and site Leaders to maintain and improve patient, provider, and staff satisfaction.

  • Supports with training and evaluating medical assistant personnel.

  • Serves as a member of the interview panel.

  • Offers professional encouragement to medical assistants, helping them develop both personally and professionally.

  • Works alongside the Director of Health Center Operations to create and execute programs for staff development, including training sessions designed to improve their knowledge and skills.

  • Assess the productivity of administration of all vaccines at Honor sites to ensure a smooth and efficient workflow. Addresses areas of concern promptly and effectively.

  • Works with the Director of Health Center Operation and site Leaders to guide staff to comply with electronic health record regulations. Ensures that patient records are accurately documented and maintained in an organized manner according to policies and regulations.

  • Reinforces that HCH vaccine policies and procedures are followed to ensure the availability of necessary vaccine stock at all Honor sites.

  • Performs other duties as assigned.  During a public health emergency, the employee may be required to perform duties similar to but not limited to those in the job description. During clinic operation shortages and without advanced notice, the employee may be relocated to a different HCH location.

What are we looking for?

  • Associate Degree required

  • 2+ years of experience managing vaccines in healthcare setting.

  • Must maintain BLS Certification through the American Heart Association.
