Honor Community Health is a 501c3 Federally Qualified Health Center co-located within 20 locations. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description

Reporting to the Director of Maternal Child Health, the Pediatric Services Coordinator is responsible for providing resources and coordination care for pediatric patients, ages 0 – 5, and their families. The Pediatric Services Coordinator will assist clinical teams in providing comprehensive care for Honor’s pediatric population, maintain relationships and growing partnerships with community agencies, address care gaps, and provide oversight for pediatric grant funding (Early Childhood Development Fund) to improve outcomes for our patients in their early developmental years. This position requires tracking and follow up of pediatric care, ensuring details are documented in the Electronic Health Record and population health tool. Development of reports, data analysis, and follow up are necessary to meet program requirements and improve quality outcomes. The Pediatric Services Coordinator will collaborate with the Maternal Child Health care team which includes family physicians, nurse practitioners, physician’s assistants, nurses, behavioral health clinicians, medical assistants, medical receptionists, community health workers, doulas, and managers, in providing quality care to pediatric patients and their families. This position is grant-funded and may be time-limited depending on grant funding renewals.

What are we looking for?

  • BSN registered nurse, or ADN registered nurse with bachelor’s degree in public health, social sciences, human services or other related field required.

  • The ideal candidate for this role has lived experience, identifies with our patient population, and is an advocate for early child development, pediatric health, and family health.

  • Two years of experience directly related to the duties and responsibilities specified is preferred.

  • Must be a registered nurse in the state of Michigan or willing to achieve licensure within three months of employment.

  • Certified Lactation Counselor training must be completed within one year of employment.

  • Must have knowledge and experience in working with the Microsoft Suite (e.g, Excel, Word, PowerPoint).

  • Experience with Electronic Health Records and documentation highly preferred.

  • Experience working in maternal child health preferred.

  • Additional language fluency in Spanish highly preferred.
