** Summary of Position Under the supervision of the Clinical Director or designee Clinical Supervisor, the Substance Use Disorder Counselor and/or Mental Health Counselor provides counseling and/or therapy to adults afflicted with the disease of substance abuse and/or dependence and/or a co-occurring disorder. Responsibilities of Position ? Screening, assessing, treatment planning, conducting education, receiving referrals, and completing care documentation. ? Conducting diagnostic assessments and providing individual and group counseling to a diverse client population. ? Creating and fostering open lines of communication for individuals with mental health and/or chemical dependency disorders. ? Preparing and maintaining a confidential, accurate, organized, and updated case file for each assigned client under appropriate supervision. ? Adhering to documentation requirements. ? Providing case management, individual and group counseling services to adults. ? Addressing clinical case disposition assignments including patient transfers, referrals and discharges. ? Completing assessments, treatment plans and progress notes for all assigned patients. ? Participating in or providing clinical information regarding patients at treatment team meetings. ? Following documentation requirements for Ohio MHAS, and other regulatory bodies. ? Maintaining knowledge of and compliance with HIPAA, policies, documentation guidelines, and other related regulatory requirements. Meeting with documentation requirements as outlined in program policies and procedures for activities. ? Assisting with intake documentation as necessary (e.g., consent for treatment, consents to release information, initial assessment, etc.). ? Ensuring client outcomes are completed. ? Ensuring that all documentation is completed in a timely manner. ? Reporting to the Clinical Director and meeting any supervision requirements. Minimum Qualifications - Chemical Dependency Counselor Assistant Certification (CDCA) or Qualified Mental Health Specialists (QMHS) - Must have at least 2 years or more of experience providing treatment to individuals with addiction and mental health illnesses. Experience working in a variety of treatment settings including outpatient settings. - Experience conducting individual, group and case management services. Knowledge of crisis intervention, multicultural and socio-economic issues. Basic advocacy skills. Ability to utilize analysis, experience, and judgment to make sound decisions. Ability to develop trust and rapport with clients and maintain confidentiality. - Familiarity with community support groups and resources - Excellent written and verbal communications skills Special Requirements - Ability to operate telephones, fax and copier. Computer skills required to use agency IT systems effectively including but not limited to the agency clinical record and Google Drive, P2, and Slack. - Must be able to work under required deadlines and schedules, while being flexible to special requests and projects. - Employee must possess a valid driver's license and be able to drive an agency owned vehicle. - Hearing and visual acuity are necessary; corrective devices are acceptable.
