Overview Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Proctor, MN consists 5 cottages/suites and up to 56 residents. High staff to resident ratio that results in more personalized care towards residents! We have full and part time available and many open shifts on block scheduling so you always know your schedule. Wage depending on experience and licensure, plus shift differentials for EVENING and NOC Shifts. Opportunities for advancement/current Lead Caregivers needed. Bonus Plan $100 after 160 hours of completed work, $100 after 320 hours of completed work, $100 after 480 hours of completed work, $100 after 640 hours of completed work. Responsibilities Performing delegated nursing and medical care designed by the RN to maintain the residents' physical and emotional well-being. Communicating with RN's and LPN's regarding relevant information pertaining the residents Accurately documenting information Benefits Paid Time Off Medical, Dental, and Supplemental Insurances 401(k) Reimbursement program available for those who complete the CNA course while employed, ask for program guidelines. Schedule: 6:30am-2:30pm, 2:30pm-10:30pm, 10:30pm-6:30am, rotating weekends Pay: Pay range shown includes shift differentials
