Title: Care Extender – ECM/ Bilingual Customer Service

Location: Rancho Cucamonga, CA (Hybrid)

Duration: 06 Month’s Contract

Pay Range $20.00 to $23.00/hr.

We are looking for a “Care Extender – ECM/ Bilingual Customer Service” to join one of our Fortune 500 clients.

Job Summary

The Care Extender - ECM acts as a crucial liaison for managing incoming referrals for Enhanced Care Management (ECM) Members, ensuring they are connected to their assigned Community-Based Care Management Entity (CBCME). This role involves screening members for ECM eligibility, managing alternative referrals when ECM criteria are not met, and using care management platforms to document outcomes effectively.

Must Have’s

  • High School Diploma or equivalent required; relevant college coursework or degree preferred.

  • Bilingual in English and Spanish , with strong written and verbal communication skills.

  • Minimum of 02 years of customer service experience , preferably in a healthcare or related setting.

  • Experience with data entry and care management platforms.

  • Must not be on a Performance Improvement Plan (PIP).

Responsibilities:

  • Referral Management: Engage with members transferred to the Care Extender queue. Conduct thorough screening to determine ECM eligibility and refer members to their assigned CBCME as needed.

  • Alternative Referrals: If members do not meet ECM eligibility requirements, assess their needs and make appropriate referrals to alternative programs such as Care Management, Behavioral Health, 3H Housing, or Community Health.

  • Documentation: Utilize various care management platforms to accurately and thoroughly document each member’s referral disposition and interactions.

  • Quality Commitment: Contribute to quality improvement and compliance efforts, including adherence to industry standards and accreditation requirements.

Key Skills and Attributes:

  • Strong organizational skills with the ability to type at a speed of 45 words per minute.

  • Proficiency in Windows applications and other relevant software.

  • Exceptional interpersonal skills with the ability to build strong relationships and resolve conflicts effectively.

  • Demonstrated leadership qualities and a collaborative approach to teamwork.

  • Ability to support management in achieving departmental and organizational goals.
