Berks Counseling Center Medical Care Coordinator in Reading, Pennsylvania

Berks Counseling Center is a recovery-oriented behavioral health, physical health, and supportive services organization providing a holistic approach to a culturally and economically diverse population for the purpose of achieving a healthy and productive lifestyle.

As a Medical Care Coordinator, you will provide advanced level administrative and client support services in the medical department. You will work closely with your direct supervisor and other department supervisors within the agency. Medical care coordination is provided to all clients who receive a medical service including psychiatric care, injections, venipuncture, IMS services, MAT services, and health and wellness services.

You will also:

  • Assist with grants, contracts, policies, procedures, insurance/billing, and other programmatic initiatives
  • Collect data and create reports
  • Collect surveys
  • Oversee the coordination of care across programs
  • Perform Continuous Quality Improvement activities 
  • Perform scheduling 
  • Assure patient access to care
  • Ensure successful coordination of care, transfer of care, and follow up care with referral agencies
  • Assure overall compliance with all CCBCH, insurance, licensing and other standards and relevant laws/regulations that govern medical and clinical care.

To qualify for the position, you will need...

  • Five (5) or more years of direct experience in similar capacity or behavioral health setting. 
  • Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.

Preferred candidates will ...

  • Have a bachelor's degree or business school certificate
  • With degree, at least three (3) years of experience working in a similar capacity and in a health or behavioral healthcare setting 
  • Have Basic Life Support certification.
  • Be Bilingual: English/Spanish.
  • List preferred skills/experiences. 
  • Have Knowledge of or experience with trauma-informed approaches.

*Physical Requirements: *

  • SEEING: Must be able to see well enough to read reports and use computer.
  • HEARING: Must be able to communicate to patients, vendors, and coworkers.
  • MOBILITY: Must have mobility to move about the agency.
  • LIFTING/PULLING/PUSHING: Must be able to lift up to 10 pounds.
  • OTHER: Must be able to write, type, and use technology.

A conviction is not a bar to employment; however, a review will be conducted to make a determination of employment should a conviction be present.
  • Receipt of original documentation and verification of relevant degrees, diplomas, certificat s, or licensure.
  • Confirmation that you are not on the OIG Federal Healthcare Program Exclusion from Participation list. Should you appear on this list, Berks Counseling Center is prohibited from employing you.

Salary Description: $38,800 to $41,100
