Providence Health Plan is calling a Clinical Care Coordinator who will: Be responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the Physician Enterprise Conduct daily work that is inclusive of ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized thru Enterprise Clinical governance Assure that an integrated approach to the patients' care coordination is fulfilled Serve as the liaison between Physician Services Enterprise providers, primary care clinics, specialist offices, patients, multiple Providence Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes Perform all duties in a manner that promotes team concept and reflects the Sisters of Providence mission and philosophy. We welcome 100% remote work for residents who live in the following area: Washington: Puget Sound Region Required qualifications for this position include: H.S. Diploma or GED equivalency 1+ years experience working as a Certified Medical Assistant with a minimum of 3+ years medical assisting work experience in a primary / specialty care setting Preferred qualifications for this position include: Bachelor's Degree 1+ years of experience working with EMR (Electronic Medical Records) systems Completion of an accredited Medical Assistant program and registered with State Board (where required by state) -AND / OR- Certified through a nationally recognized Medical Assistant program such as AAMA, AMT, RMA, CCMA or NCCT -OR- Nationally Certified Pharmacy Technician or registered with the State Board Why Join?
