Providence St. Joseph Health Clinical Pharmacist: Pharmacotherapy in Richland, Washington
Description Under the general direction of the Pharmacotherapy Clinic manager with matrix direction provided by Director of Pharmacy, Practice Administrator/Manager and/or Provider, performs all necessary functions to ensure optimal use of medications to achieve specific outcomes that improve a patient's quality of life. The Clinical Pharmacist works collaboratively with the interdisciplinary team to provide chronic disease state management (e.g., diabetes, hypertension, asthma, COPD, CHF, hyperlipidemia and poly-pharmacy) to patients. These services include initiation, modification and discontinuation of medication therapy based on patient history, medical records and barriers to access. It also includes monitoring medication therapy for side effects, effectiveness, and allergies. Each of these steps of the patient care process must be documented appropriately within the electronic health record. Pharmacists are an integral part of providing drug information to Kadlec Clinic patients and professional staff. In addition, these services include the provision of other medication care service activities as deemed necessary by pharmacy leadership. Required Qualifications: Bachelor's Degree: Graduate from an accredited School of Pharmacy with a BS/PharmD degree or equivalent education/experience Washington State Pharmacist License upon hire One (1) year of PGY-1 residency or equivalent experience Preferred Qualifications: Two (2) years of PGY-2 AmCare residency or equivalent experience Certification in Ambulatory Care upon hire Why Join Providence?
