East Bay Community Action Program Case Manager, Older Adults and Adults with Disabilities in Riverside, Rhode Island

East Bay Community Action Program is hiring for a Case Manager, Older Adults and Adults with Disabilities for our 100 Bullocks Point, Riverside, RI location. The Case Manager is responsible for providing person-centered, unbiased care coordination and case management services to aging and disabled (AED) population, as well as to adults living with intellectual and developmental disabilities (I/DD). This role involves assessing client needs, developing Person - Centered care plans, coordinating community services, and ensuring comprehensive documentation. The Case Manager works closely with clients, families, and interdisciplinary teams to promote client well-being, maintain compliance with regulatory guidelines, and ensures that all services are delivered in a respectful, confidential, person - centered and culturally competent manner.

Responsibilities:

  • Coordinate care and services for aging and disabled (AED), and adults with intellectual and developmental disabilities (I/DD)
  • Conduct comprehensive assessments of clients needs, strengths, and resources.
  • Develop, implement and monitor person - centered care plans, addressing medical, social, and community service needs.
  • Provide ongoing case management and respond to clients inquiries.
  • Connect clients to additional services and supports.
  • Ensure compliance with Conflict - Free Case Management (CFCM) regulatory standards and documentation requirements for Cost Share and Medicaid AED and I/DD clients.
  • Investigate and address Adult Protective Services (APS) cases, ensuring the safety and well-being of vulnerable clients.
  • Report abuse, neglect, or exploitation cases to appropriate authorities.
  • Maintain confidentiality and provide culturally competent, person-centered services.

Qualifications:

  • Bachelor's degree from an accredited institution in fields such as Counseling, Education, Gerontology, Human Services, Nursing, Social Work, Psychology, or a related field is preferred.
  • At least two years of relevant experience working with low-income elders and individuals with disabilities.
  • Valid driver's license, automobile insurance, and reliable transportation required.
  • Willingness to travel throughout Rhode Island.
  • Knowledge of human behavior and the aging process.
  • Ability to work effectively with people from diverse cultures and backgrounds in a supportive capacity.
