Responsibilities

Primary Care Rochester and General Pediatric and Adolescent Medicine are jointly seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary team within both Community Pediatric and Adolescent Medicine (CPAM) and General Pediatric and Adolescent Medicine (GPAM). The position will be shared 50/50 between the two divisions.

Community Pediatric and Adolescent Medicine offers the opportunity to demonstrate leadership and creativity within the challenges of evolving health care delivery models. Our Nurse Practitioners and Physician Assistants are supported within a dynamic care team and encouraged to contribute to improving the health of our pediatric population in many ways. Our team has low turn-over, supportive physician colleagues, and strong role models. You would have the opportunity to provide acute, chronic, and preventive health care services to pediatric patients in the outpatient clinic setting. The provision of care includes, but is not limited to, comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients. Our work is challenging and rewarding, we are experimenting with care delivery models that provide opportunity for creative healthcare solutions. We are focusing on transforming care delivery to sub-set populations; complex pediatric patient care, adolescents and those with mental health concerns, and those with social determinants of health risk because of poverty, culture/language barriers, or refugee status. Share your creativity and embrace leadership opportunities by joining us in making a difference for our pediatric patients.

The General Pediatric and Adolescent Medicine (GPAM) Division is a front-door practice for pediatric patients and their families seeking highly coordinated, integrated evaluations for complex, serious, and chronic conditions including rare disease and autonomic disorders. A majority of GPAM patients are seen in a consultative model where our providers coordinate evaluations with our pediatric specialty teams and leverage cutting edge diagnostic technology and services to accurately diagnose disease and develop optimal management plans. For children requiring longitudinal specialist care at Mayo Clinic for their highly complex and rare disease, our GPAM providers also serve as care coordinators through our complex care clinic. Providers in our complex care clinic work closely with key care team members including complex care nurses, pediatric pharmacy, and social work. The goal of our complex care clinic is to improve patient outcomes and experience while helping to simplify and streamline care. We are seeking a NP PA to join GPAM with a focus on supporting the complex care clinic. The role will be primarily outpatient based serving established patients. The role will also have a strong emphasis on quality improvement and patient and family experience in complex care. We are seeking dynamic, collaborative, and innovative candidates who wish to join GPAM to help us continue to transform the care of children with medical complexity.
